The Enrollment Department then transfers all the information from the Tribal Roll to the Deceased Roll. Designation Form. Once the minor child turns eighteen (18) they must then complete the form designating who their beneficiary will be. B. Payment of a Beneficiary Siletz Tribal Death Benefit Policy I- Purpose The purpose of this policy is to provide guidelines for the Death Benefit Insurance available to all Siletz Tribal Members. 11. Definitions Beneficiary Burial Expenses Check Request Death Beneficiary Form Death Benefit Death Benefit Insurance Death Benefit Designation Form Death Benefit Report Designated Beneficiary Maximum Allowable Amount Removal III. * See Designated Beneficiary. * Cost incurred as a direct result of the burial of a deceased enrolled Tribal Member. * A written request for a check to be processed in the name of the Beneficiary. * See Death Benefit Designation Form. * See Death Benefit Insurance. * Also referred to as Death Benefit, is the money that is paid to the Designated Beneficiary upon the death of an enrolled tribal member. * Designates the beneficiary and includes a beneficiary's name, address, and telephone number. This completed form Is signed by the enrolled tribal member with their name, roll number, date of birth and the date the form was completed. * A report lists deceased tribal members for the current fiscal year. * The individual appointed by a tribal member to receive the Death Benefit Insurance. * Maximum monetary compensation to satisfy a single death benefit claim. * Process of removing a deceased enrolled tribal member's name for the current Process A. Death Benefit Designation Form 1. Completion of Death Benefit Designation Form Once a tribal member is enrolled a Death Benefit Form is sent to them with notification of enrollment. This form is for the tribal member to designate a beneficiary. It is the tribal member's responsibility to complete the form and return it to the Enrollment office as soon as possible. The beneficiary must be over 18 years of age. 2. Changing The Death Beneficiary An enrolled tribal member may change their beneficiary at any time. A new Death Benefit Designation Form must be submitted to the Enrollment Office. The tribal member must also complete and return a new form if the Designated Beneficiary's address changes. The Confederated Tribes of Siletz and the Enrollment Department are not responsible for lost or misdirected forms. 3. Minor Children. A legal parent or legal guardian that has custody of the minor child may designate someone and complete the Death Benefit addressed stamped envelope with which to return the receipt. 5. Removal From the Roll Removal from the roll is posted when the receipt is returned. Action is taken by Tribal Council in the form of a resolution ZZZZZZSZ2ZZZ2Z2a5^Z2ZZZZZ233EZ2ZZEB &3SB22: 1. Documentation It is the responsibility of the designated beneficiary to forward a copy of the Certified Death Certificate upon the death of an enrolled tribal member and ensure the Enrollment Department has a correct address and phone number for them. An invoice from the funeral home must also be provided. In the event that there was no beneficiary designated then a family member could be responsible for providing a death certificate and invoice. 2. Processing Payment When the Enrollment Department receives a copy of the death certificate and an invoice from the funeral home they will immediately make out a check request to the funeral home for the funeral expenses up to the maximum allowable amount. Additionally, a check request will be made out to the designated beneficiary for the remainder of the maximum allowable amount, if there was a beneficiary designated. 3. Death Benefit Report The Enrollment Department will complete a Death Benefit Report. This report lists the roll number, name and date of death of every deceased tribal member for the current fiscal year. A copy will be sent to the Administrative Manager, and the Bureau of Indian Affairs. 4. Issuing Payment Once the checks are received in the Enrollment Office, copies will be made, and the originals will be sent out by certified mail along with a letter explaining the purpose of the check, a receipt that the recipient must sign and return, and a self Remember Our Veterans by: Teresa Miller Memorial Day will soon be here and with it's arrival we honor our veterans, in one of the few ceremonies held in Lincoln County. On this day let us humbly remember as Billy Rae Cyrus puts it, "Some gave all". Of those that were fortunate enough to come home-some were honored and received with open arms, some were not, depending on which war was taking place and their community. No matter where or when these brave men and women served their country, their lives were deeply affected through their service to our country. In turn, the lives of their families were also, whether good or bad. Many have struggled for years to overcome the lasting effects of combat, while some have excelled as a result of their military background. All deserve recognition for their sacrifices, whatever they may have been. Because of these fine individuals we benefit in ways we too often take for granted. When you watch the sunset in the evening, hear a baby laugh, smell the fragrance of a newly cut lawn or feel the wind gently on your face, remember who made this possible and that "Some gave all".
