To be entered on the register the patient must appliy the following information: 1) Name, address and phone number and diagnosis, 2) date of diagnosis, 3) name, address and phone number of physician, 4) estimate of cost; surgeon, hospital and laboratory. We cannot gurantee funds will be made available for all procedures but we want to try. Eligibility of Non-Indian Health uses for II1S Services: On Januray 6, 1978 the Director of the Indian Health Sercie ordered that all IHS services, contract and direct, would be made available to an non-Indian spouse of an eligible Indian. This was rescinded on April 10, 1978. For the present non-Indian wives of eligible Indian husbands and their dependents may be provided direct and contract health care. No IHS services will be provided to non-Indian male spouses after April 10, 1978 until a properly implemented policy is established. Commitments and care begin January 6, 1978 and April 10, 1978 to non Indian male spouses may be completed but no new care may be started. You will be kept advised of changes as they develope. ♦Community Health Representative: The Confederated Tribes of Siletz Indians will be hiring one (CHR) Community Health Representative by July 1, 1978. This individual will be based at Siletz but provide services to the north Lincoln, Polk, Yamhill, and, Tillamook counties. The Community Health Representative (CHR) will serve as a link between the Siletz Indian people and health providers. Training will be provided by Indian Helath Service. Training will be based upon the needs and interests of CHR. This program is funded by the Indian Health Service through a contract with the Confederated Tribes of Siletz. CHR must have education, motivation, and experience which demonstrates his or her ability to work in an Indian correnunity. He/She must be able to establish and maintain a report, to gain and maintain the confidence of the local population, to grasp and understand the health problems of the people and to interest family units/individuals/groups to actively participate in programs offering curative and preventive health care. CHR needs a working first hand knowledge of Indian attitudes CHR must have valid driver licenses have a vehicle to be used on the job, and acces to a telephone. CHR must be able to write reports. CHR must maintain patient confidentiallity. CHR must be willing to participate in basic on-going training opportunities. Applicants will be interviewed by the Tribal Health Committee June 28, 1978. The following health services for the Siletz Indians in the eight county area are tentatively set for these months: Pharmacy-June, Hospitals-July, Physicians-August, and Dental in October. You must have an application on file at the Chemawa Indian Health Center with approval before these services can be rendered. The eight counties of eligibility are Lincoln, Lane, Linn, Benton, Polk, Yamhill, Marion and Tillamook. MEETING Seventy people were present at the June 3 meeting in Siletz. The next meeting will be July 1. There will be a potluck lunch starting at 11 A.M. Everyone pleasecome. Bring something for lunch, visit your friends. Stay for the meeting and help clean up the hall. Any items of interest to all of the Siletz Indians will be accepted and printed as space i3 available.
