Transportation
Community Health Transportation purpose is to ensure Siletz Tribal members in meeting their medical, dental
and mental health non-emergency transportation needs and is provided as a means of last resort. (This is ex-
cerpted from Siletz Community Health Clinic Policy §Part 11B.)
Who does Community Health Staff Transport:

Siletz Tribal Elders

Siletz Tribal Members with impaired mobility

Siletz Tribal Member Minors who must be accompanied by parent/legal guardian

Siletz Tribal Members who lack personal vehicle or other transportation sources such as family, friends and
public transportation

leave detailed message and CH Staff will return phone calls to confirm availability.
