Transportation
Community Health Transportation purpose is to ensure Siletz Tribal members in meeting
their medical, dental and mental health non-emergency transportation needs and is provided
as a means of last resort. (This is excerpted from Siletz Community Health Clinic Policy §Part 11B.)
Who does Community Health Staff Transport:

Siletz Tribal Elders

Siletz Tribal Members with impaired mobility

Siletz Tribal Member Minors who must be accompanied by parent/legal guardian

Siletz Tribal Members who lack personal vehicle or other transportation sources such as
family, friends and public transportation

ability.
