Johnson family wins top 10 national workplace safety award for Sert-A-Plug J3 Group LLC is proud to announce that its patented Sert-A-Plug received its first safety award as a top 10 finalist in the Occupational Safety and Health Administration (OSHA)/National Insti- tute of Occupational Safety & Health (NIOSH)/Mine Safety and Health Administration(MSHA) 2016 Hear And Now Noise Safety Challenge Awards held in Washington, D.C., on Oct. 27, 2016. This event provided opportunities for 10 inventors, selected from 28 submis- sions, to travel from all over the country and Canada to pitch their solutions for reducing hearing loss from workplace exposure. Civil servants from partnering federal government agencies worked with the entrepreneurial community to launch the ‘Hear and Now – Noise Safety Challenge’ on Aug. 1, 2016, with the dual goals of inspiring creative ideas and raising busi- ness awareness of the market for work- place safety innovation. On Oct. 7, they selected the top 10 solutions from 28 submissions to chal- lenge.gov. These solutions were selected because they show potential for better protecting the 22 million workers exposed to hazardous noise every day. NIOSH will feature Sert-A-Plug with John Johnson and his two sons, Shawn and Joshua Johnson, for The NIOSH Science Blog (45,000 subscribers). NIOSH and OSHA officials were impressed with Sert- A-Plug. This is Sert-A-Plug’s first feature in an industry forum. Missing Moccasins What is a “Missing Moccasin”? This is a Tribal member who has an invalid address on file with the Siletz Enrollment Department. The Enrollment Depart- ment or another department has received returned mail for the Tribal member marked as undeliverable. Enrollment staff will attempt to contact the Tribal member at the most recently provided phone number. Once it is determined there is no way to get a message to the Tribal member, he/she will be added to the “Missing Moccasin” list for publication in Siletz News. In order to receive any mail from the Tribal administration building, including per capita checks, 1099-Misc tax form, Tribal election information and Siletz News, the Tribal member must submit his/her address change on the Enrollment Department’s Address and Contact Infor- mation Update form. For security reasons, address changes are no longer accepted over the phone. As of Nov. 10, 2016, there are 56 ADULTS and 94 CHILDREN who are Missing Moccasins. If you are on the list of adult Missing Moccasins below, please update immediately. If you are not receiving the quarterly statement of your child’s trust account, they may have an invalid address on file. Children’s names are not published publicly. If you know someone on this list, please ask them to contact the Enrollment Department to have a form sent to them. The form is also available on the Tribal website at ctsi.nsn.us under Government, then select Enrollment and the second tab has all of the Enrollment forms; or get one at any of the four Tribal offices. If you are unable to make it to the administration building in Siletz to obtain or replace your Siletz Tribal ID card, you are now able to request one at of the three area offices in Eugene, Salem and Portland. They will take your photo and obtain your signature, then send it to the Enroll- ment Department in Siletz. Your Tribal ID will then be mailed by CERTIFIED mail to your address on file or to the area office where you made your request. You can request your Tribal ID online! If you do not live in the 11-county service area or are not able to make it to a Tribal office during work hours, there is a form on the Tribal website under Govern- ment Listings > Enrollment and the tab marked Enrollment Forms. Tribal members can print the form titled Siletz Tribal Identification (ID) Request and follow the directions regarding the digital photo specifications, identity verification and signature verification, then mail the form to the Enrollment Department. The Tribal ID machine is based in Siletz at the administration building. Staff is available Monday through Friday from 8 a.m. to 4:30 p.m. to issue a Tribal ID card on site. There are times when the ID machine is out of the office traveling to other offices and this information is in Siletz News as well as the front desk knows when the ID machine is not available. If you have not updated or replaced your Tribal ID card since April 1, 2016, there is a new two-sided format with additional information that makes it more acceptable as a form of identification (see sample below). The expiration date is 10 years from the issue date. Once we have issued the new two-sided ID card to you, we can mail you an updated card when you change your address when you request one. Once you have received the new Tribal ID card after April 1, 2016, if you need to replace your card with no changes (for example, an address or name) within the same calendar year it was issued, there is a $5 replacement fee (per Tribal Resolu- tion #2016-087). You can obtain up to one new Tribal ID card per calendar year at no charge. Call the Enrollment Department at 800 922-1399, ext. 1258, or 541-444-8258 if you have questions about this fee. Enrollment Committee vacancy Are you interested in your family history and how in many ways We Are All Related? Have you considered vol- unteering your time for a Tribal Standing Committee? There is a vacancy on the Enrollment Committee. Meetings are once per month from January through November, generally on Wednesdays at 3:30 p.m. in Siletz. Mile- age (with a limited distance per the Stand- ing Committee Ordinance) and a meeting per diem is paid for attendance.
