Where is my per capita check? Per Tribal Council Resolution 2015- 169, per capita checks were mailed July 31 to the address on file with the Enroll- ment Department. If you were a “Missing Moccasin” as of July 17, your check will be held in the Accounting Department until a correct address is submitted. Policy states you must wait 15 business days (in the case of per capita checks until Aug. 21) from the time of mailing before you can submit a request for a “stop payment” and have the check re-issued. This allows time for the check to be returned if it is “undeliverable” at the address to which it was mailed. In order to have your check re-mailed, you must have your current mailing address on file with the Enrollment Department. How do I update my address on file with Enrollment? In order to receive any mail from Tribal administration, including per capita checks, the Tribal member must submit the Enrollment form “Address & Contact Information Update.” Address changes are not allowed over the phone. The form is available by: 1. Going to the Tribal website – ctsi. nsn.us, click “Government Listings” on the banner, select “Enrollment” from the drop down and the form is on the right side of the page. You can fax or scan/email the form, then mail the original to get your contact information submitted quickly. If you are not receiving your child’s quarterly statement for his/her trust account, he/she may have an invalid address on file.
