Enrollment Department details to help get what you need Changing my name distribution. Contact the trust officer or the Enrollment Department. If you are changing your name, you must submit the Information Change form with docu MilIO mentation and a copy of your Social Security card confirming your new name. Documentation includes marriage certificates, divorce decrees and court orders. w w Calling The Enrollment Department receives calls regarding information for other Tribal departments. You can call the Tribal receptionist at SOO- 922-1399 or 541-444-2532 and request * the following Tribal departments: Changing my address during per capita ( If you are changing your address for your per capita distribution check, this must be done in writ ing with your signature, roll number and date of birth, addressed to the Enrollment Depart ment. This is to avoid fraud. Accounting for per capita documentation, W-2, W-4, 1099 and other monetary information C Tribal Court for garnishments, name decrees and other legal information C Contract Health Services for out- of-area medical benefits and other medical information To avoid delay of your per capita distribution check, update by July 13, 2012. Applications If you are submitting an application for enrollment with the Tribe, please make sure: If you do not want to receive distri bution. you can disclaim your right to Tribal Identification Card * The application forms are complete; AND Include the applicant’s Social Security card; if the applicant is a minor in the process of applying for his or her Social Security card, it can be sent to the Enrollment Department at a later date; AND Include the state-certified birth certificate, preferably the long-form version; OR If the applicant is claiming an ancestor and the applicant’s natural parents are not enrolled, then include the state-certified birth or death certificates proving the direct ancestor who is named on Tribal membership rolls or records We only accept originals. We return all originals via certified mail. If necessary, Tribal staff may make copies of original documents for the applicant, including the area offices. This distribution will be made to all eligible Tribal members who are alive and enrolled as of July 1, 2012. All Tribal members eligible for distri bution who are at least 18 years old as of Aug. 1, 2012, will receive their distribu tion by mail. Before your distribution can be mailed, the Enrollment office must have a copy of your Social Security card on file and a valid address. The deadline for updating your address for this distribution is July 13, 2012. All Tribal members under age 18 will have their distribution held in their minor’s trust account. If you have military service photos, the Enrollment staff will be available to scan the photos on the same dates in July and August that we're doing picture IDs (see left). Gathering these materials is the necessary first step in our basket-making process. Hazel sticks (kvn) generally are ready to pick and peel in the middle of spring. Hazel from areas in and around the Wil lamette Valley generally is ready a couple of weeks earlier than hazel found near the coast. Many variables help determine the best time to pick hazel. Beginning in April, hazel should be checked to see how easily the bark peels. If hazel is picked too early, the bark won’t slip and has to be scraped off. This is labor-intensive and also takes the natural sheen off the sticks. Hazel is the foundation for most Siletz baskets. When gathering, most people pick a range of sizes, peel them and then grade them for length and diameter. They are then bundled and stored for later use. Spruce root (xii-wan-tr'e) is the other main material used in our baskets. It can be gathered year round. An area with spruce trees is found and the roots are dug and rolled up. They then are boiled until the outer layer or membrane will slip off. The roots then are graded and stored for later use. Remember, there is a short window for gathering hazel sticks, usually 30 to 40 days depending on weather conditions and other factors.
