Nehayla Jade Cordova

We would like to announce the birth
of our daughter, Nehayla Jade Cordova,
on Sept. 11, 2011, at 2:31 a.m. She
weighed 6 pounds, 10 ounces and was
19 inches long.
Proud parents are Sarah Butler and
Jesse Cordova, and proud big sister is
Tenaya Cordova.

Kai RunningBear Williams (Kai Bear) ruptured our tear ducts by arriving at
5:51 p.m. on Oct. 4, 2011. He weighed 8 pounds, 10 ounces, with each cheek being
at least a pound.
We love you so much and thank you for your arrival in our lives.

What is LIHEAP?

Assistance Program) is a federally
funded program that helps low-
income households with their home
energy bills.
LIHEAP can help you stay warm
in the winter. By doing so, you can
reduce the risk of health and safety
problems (illness, fire or eviction).
The residence must be located within
the 11 -county service area.

Recent copies of utility bills (the indi­
vidual named on the utility bill must be
included in the household composition)
Final utility termination notice (if
you’ve received a shut-off notice from
your energy company)
The applicant or a child residing in
the household must be an enrolled Siletz
Tribal member.

What information do I need to
apply?
To participate in the program, you
must submit the following:

Documents verifying the income of
every household member age 18 and
older, such as:
♦ A recent payroll stub or other
proof of current gross income
♦ Documentation, such as an award
letter, showing income from
Social Security, unemployment
insurance, pension funds,
disability, etc.
