Tuesday, Dec. 3 Wednesday, Dec. 4 Thursday, Dec. 5 9 a.m. 9 a.m. 9 a.m. 9 a.m. - 3 Salem p.m. p.m. p.m. p.m. Monday, Dec. 16 Tuesday, Dec. 17 Wednesday, Dec. 18 2 p.m. - 7 p.m. 9 a.m. - 7 p.m. 9 a.m. - 5 p.m. If you plan to be a walk-in and expect to receive services, please bring the following: Copy of Tribal ID or Certification of Indian Blood, also a picture ID for head of household Copy of Social Security cards for all family members on the application C opy of documentation for one full month of income for any person 18 years or older and employed Copy of documentation of benefits received, including pensions, Social Security disability, child support, or any unearned income received If any person on the application is 18 years or older and not receiving any income, this person must complete a Zero Income packet. This packet must be completed by someone not related to the person claiming zero income, also not living in the household. Example: neighbor, friend, landlord, or employer If you’re living rent-free, you must provide a statement from your landlord. If you live with someone not on the application, we need a statement from someone outside the household not related to you stating that you keep and prepare meals separate. We must receive all this information prior to scheduling an appointment for you. Please be advised that you will not be serviced until USDA receives all documents. Caregiver Conference A second Caregiver Conference has been scheduled for Dec. 10-11, 2002. This conference will present more hands-on training than the previous one. It is for anyone who is a caregiver for a tribal elder or disabled member. If you would like to attend this conference or are interested in being trained to be a “caregiver trainer,” please contact Kathryn Dick or Elouise Case at 1-800-922-1399 or 541-444-2532.
