ILLNESS
Cover* every accident, no matter where or how it
happens.
Pays indemnity for both total and partial disability
of from one day to two years.
Large indemnity for loss of life, one or both limbs
or eyes, by accidental means.
Insurance effective immediately after issuance of
policy.
Double indemnity for travel accidents.
Surgeon’s fees—non-disabling injuries.
Identification and registration.
Covers at full rate of indemnity every disease com­
mon to both sexes.
One-half indemnity for non conflnement.
lays for first week if insured is disabled twenty-eight
days.
Disability limit—six months.
Accidental death and dismemberment indemnity is
increased five per cent, each month until original
amount is increased fifty per cent.
Illness and accident disability benefits are increased
ten percent, if premiums are paid annually in advance;
nve percent, if paid semi-annually in advance.
INDEMNITIES—$40 00 per month for disability from either illness or accident; $400.00 Accidental
Death and Dismemberment Indemnity.
COST—/ Age not exceeding forty-five—the initial or policy fee of »3.00, which pays for the first
month s insurance; 11.25 per month thereater.
Increased indemnities for a proportionate increase in
premium.
