Diagnosis of Pernicious
Anemia
A numer of people write
to say that they have a rela
tive who has pernicious ane-
I m i a; they
them selves
have an ane
mia, and they
want to know
how they can
tell if it is of
the p r 1 mary
(Add isonian)
type, and not
a sec ondary
type. The sec
ondary type is due to a loss
of blood, as by bleeding piles
in a man, or excessive men
etruation each month in the
case of a woman. Primary ane
mia is due to something hered
itarily wrong with the blood
forming organs, such as the
bone marrow.
If long ago my father or
mother had died of pernicious
anemia, I certainly would
have my blood examined ev?
ery year, and examined by an
expert who could recognize
ty a glance at a "stained
smear" a tendency to enlarge
ment of the red blood cells
and a tendency for each cell
to contain more hemoglobin
(red coloring matter) than nor
mal. Also, I would watch out
for feelings of numbness or
tingling or weakness in my
legs, due to the nervous le
sions that sometimes accom
pany pernicious anemia.
' I would also have someone
pump my stomach to see if
I had a lack of acid in my
gastric juice. If 1 had no acid
in my stomach, I would know
I had probably inherited a
tendency to pernicious ane
mia. .Hut, u I naa pieniy ot
acid, I would assume that I
was one member of the fam
ily who had not inherited the
. tendency. With a tendency to
pernicious anemia I might
' have become gray in my 20s
or 30s. If I had no gastric acid
I would take a tiny dose of
Vitamin B12 once every two
or three months. It is remark-
. nble what an extremely small
dose of vitamin B12 will keep
' one from getting into trouble
with pernicious anemia.
If I had a secondary anemia
I would not take B12, 1 would
! take iron. Unfortunately, in
college we doctors were not
well trained to do this, and
hence all the expert blood men
. in the country are constantly
complaining that most of us
' doctors give B12 when we
ought to be giving iron. They
also tell us that no one needs
s shot of B12 every day or
even every week
I was just reading an arti
1 cle by an expert who says that
' still the best iron, he thinks,
is the sulfate which can be
t?kcti by moth every day. All
one needs is a small pill con
taining it.
Drugi at Start of Pregnancy
The terrible experience of
many pregnant women who
took the new drug thalido-1
mide should cause all women
who have just become preg
nant, or who have Just missed
a period, to stop (for at least
two months) taking any drugs
that they happen to be taking,
und especially any new drugs
which have recently come on
the market, and hence might
have undesirable "side - ef
fects" as yet not known.
I am writing this column
because already, in the great
amount of mail which I re
ceive, there are letters from
women who say, "I am preg
nant, and 1 am taking a new
tranquilizer ror sleep-maker,
or reducing drug, or drug to
lift me out ox a depression,
etc.) will it do me any

harm or will It do my baby
any harm?"
Rather than trying to an
swer all these many letters,
I will say here that a woman
just pregnant will de well to
take no drugs. After a couple
of months her baby will be
so nearly formed in all its
parts that it will be much less
likely to be injured by a drug
or by an attack of German
measles.
Bursitis
Many people ask. What is
a bursa? It is a little bag of
fluid which serves often as a
little pulley. A tendon goes
over it. Why does it become
inflamed? I think because the
person has a tendency either
to arthritis or the much mild
er, but very common, fibro
sitis. (This disease is describ
ed in my booklet on Arthritis.)
The commonest bursa that
gives trouble is in the region
of the shoulder. Sometimes
pain in it calms down quick
ly by itself, sometimes it is
helped by physiotherapy (heat
and massage); but sometimes
it doesn t show any signs of
clearing up under such treat
ment. Then X-ray treatment
will promptly give relief in
two out of three cases.
In past years, "Surgeons used
to cut out bursas that filled
up with lime salts, but they
quit operating when they
found that the little sacs gen
erally became normal again,
often without any treatment.
I remember once when a bur
sa on the end of my left elbow
became as big as a walnut
and so sore I was all set to
go to a surgeon and have it
cut out. And then, in two days,
it disappeared all by itself.
If you have a painful joint,
you will want to read Dr.
Alvarez' booklet, "Arthritis
and Rheumatism." You may
get it by sending 25 cents and
a self-addressed, stamped en
velope with your request for
it to Dr. Walter C. Alvarez,
