of mankind is the hemorr
hoid (pile), which is a dilated
vein located
In the anal

outlet of the
large bowel.

people, espec
ially past 40,
have piles
which do not
bother' them,

ly coming down or occasion
ally filling with a painful
mood clot, or are bleeding
badly. Some persons have to
be operated on because they
bleed until the amount of
hemoglobin (red coloring mat
ter in the blood) gets down too
low for good health.
Some people get by until
they get badly constipated, or
they get diarrhea. It is prob
able that heredity plays a part
in producing piles in some
people. Many women get
their piles during pregnancy
when, perhaps because of the
presence of the much-enlarged
womb, the large veins In the
pelvis or lower abdomen are
pressed on. r.
Piles generally are divided
into the Internal" and "ex
ternal" types. The internals
can be out of sight - inside of
the rectum - while the ex
ternals are protruding out
side. When the plies bleed,
blood is found usually on the
toilet tissue. Always, when
a person has bleeding piles,
he must remember r especial
ly If he is over 40 - that he
can easily have, In addition,
a cancer of the rectum. This
usually can be felt or seen by
a doctor if he will only ex
amine with his finger or with
a lighted tube, called a proc
toscope. Can Be Painful
Piles often give trouble
when they prolapse, or come
out, especially when the per
son strains at stool, or coughs,
or sneezes. Then the person
may have to push them back.
The piles are small, rounded,
purplish, usually soft, masses
- enlarged veins filled with
blood. A pile can become

in it clots. This can happen
when the person is constipated
and a hard mass of excrement
perhaps tears the side of the
pile.
As I said, in most cases,
piles do not have to be treat
ed; they give trouble only
occasionally. Sometimes, if
the piles are hurting or Itch
ing, the patient can use a
soothing suppository, such as
can be found in drug stores.
Some physicians treat piles
by injecting into them with a
hypodermic needle an irritant
drug which is designed to
cause an inflammatory re
action. With this inflamma
tion there is a possibility that
the walls of the pile will stick
together, and this will put an
end to the trouble. But this
doesn't always happen, and
occasionally, If the Injection is
poorly done, or if the person
is highly sensitive to the in
jected drug, the tissues will
slough.
I have seen nasty ulcers
produced in this way. Be
cause of these accidents, many
expert proctologists (experts
in this field (refuse to use the
injection type of treatment. If
injections are tried, they may
have to be repeated several
times during a week or two.
The treatment should never
be given by anyone who is
not expert in this field be
cause if he injects in the
wrong place, he can easily
get a slough and a bad ulcer.
Sometimes, if the patient Is
In great pain, due to a pile in
which the blood has clotted, a
cut under local anesthesia will
let the clot out, and this will
give some relief. The only
treatment advisable, especial
ly for large piles, is tleing
them off and taking them out.
There are several types of
this operation. It is important
that the surgeon who does this
work Is skilled in it, because,
otherwise, it is easy for the
patient to get an uncomfort
able result. When, a person
has suffered greatly from
hemorrhoids, a satisfac t o r y
operation can give tremen
dous relief.
Early Gray Hair
Following the appearance
of a column a while ago on
early gray hair and Its pos
sible significance, I have had
letters from many people who
want to know again what they
should do. For Instance; a
number of women have told
me of their teen-age girl who
is getting gray. My advice is
to have a , blood specialist
study the blood to see if there
any sign of a primary
anemia, such as used to be
called pernicious anemia. I
would also want the stomach
pumped to see If the girl has
any free acid in her stomach.
If she has plenty of acid,
she probably will never have
primary anemia, but if she has
no free acid, then she may be
headed for a severe anemia.
In that case, the family doctor
may want to start her on regu
lar dosage with, perhaps, Vita
min B12. If she needs it, she
will need it for the rest of her
days. There is another thing
