Hundreds of people ask me
what to do about "colitis"
and I cannot make any sug-
g e s t ions be
cause I cannot
guess what
kind of colitis
the individual
has. It is most
u n f o rtunate
that many
years ago we
doctors got in
to the bad
uu:4 t Mnii
UR. ALVAREZ "l t""
Ing three very different con
ditions by the same name. We
even use the Greek possessive
ending "itis" under a miscon
ception. Originally, when the
ancient physician said "coli
tis," he added after the Greek
word for "disease." What he
said then was "disease of a
colon." But, with the passage
of time, the word "disease"
got left off, and doctors who
did not know Greek assumed
that the ending "itis" (of the)
meant inflammation. Hence,
today, arthritis has come to
mean inflammation ot a Joint,
and appendicitis ha come to
mean inflammation of the ap
pendix. This mistake did not do any
harm, but then, as I have
pointed out before, every so
often a new and popular diag
nosis has to be devised to sat
isfy quickly the thousands of
neurotic women wl.o only get
angry when told me iruin
about their nervousness, and
for a while we doctors chose
for this purpose the word
colitis." It soon caught the
fancy of millions of women;
who liked it; but this was un
fortunate because "colitis in
dicates a serious inflamma
tion of the colon, which these
women never have.
Diagnosis Med Up
The naming of "pure
nerves' or hysterics or the
result of annoyance with a
husband or mother-in-law. as
"colitis" messed up medical
diagnosis badly enough; but
then we doctors got into an
other bad habit: wa got to
giving this name "colitis" to
a common nervous trouble of
overly sensitive men and
womcn-a trouble sln which,
when tha person goes out to
supper, he or she fills up with
gas. If this gas cannot quick
ly be passed, the person's
bowel Is soon cramping down
painful,, and much mucus
with a little watery fluid
starts forming In the rectum.
Many people call this "diar
rhea," but it Is not a true
diarrhea because, during the
spell, there Is so much spasm
in the lower end of the bowel

that no fecal matter can come
down.
A few young women even
have trouble marrying be
cause this so-called mucous co
litis makes them fearful ever
lo go out with a man. in these
cases, the colon, even after
years of this formation of gas
and mucus, remains to all ap
pearances perfectly normal.
Hence, there is no colitis,
and this name should not be
given to the condition. I have
always refused to use "colitis"
in this sense.
Tru Colitis
Finally, there is a true co
litis: the so-called "chronic
ulcerative colitis" in which
the person really has a badly
Inflamed large bowl, and can
become seriously ill with a
fever, with many bloody and
watery bowl movements each
day. This disease can easily
be diagnosed from the history,
the x-ray studies, and a look
into the bowel with a lighted
tube. Because we do not know
the cause, and have no good
and specific treatment for this
disease, it is often hard to
treat. In many cases, rest in
bed, diet, cortisone-like drugs,
and certain antibiotics will
help. Psychic strain can cause
flare-ups in the disease, ana
hence I always feel rather
hopeless when the patient is
kept constantly upset, perhaps
by an unhappy marriage.
I hope now that the thou
sands of people who do not
know which of the three types
of "colitis" they have will un
derstand when I do not an
swer their letter. Obviously,
I cannot suggest any treat
ment when I cannot guess
what is wrong.
