For some 45 years I have
been going up and drrvn this
land pointing out that a con
siderable per,
centage of the
patients found
in doctors' of
fices and in
hospitals have
more of a psy
c h i c trouble
than a physi
cal one, such
jksj as they think

they have
perhaps in their uncomfort
able abdomen. I gave my first
talk on the related subject of
psychosomatic medicine in
1912, before the San Francisco
County Medical Society.
For years, also, I have been
suggesting to my medical
brethren that a large percent
age of the persons who are
looked on as just nervous are
really more nearly psychotic
than nervous. The compul
sions and phobies and attacks
of hysteria suffered by these
patients, or their symptoms
of schizophrenia, or their de
pressions or ideas of persecu
tion suggest to me a mild
psychosis.
Usually, when, I show the
patient to one of my psy
chiatric friends he agrees with
me that I am dealing with a
psychosis. Many of the pa
tients a diagnostician or in
ternist specialist sees every
week are what some of his as
sistants will call "nice screw
balls." They were always ec
centric and add, and many
will probably always remain
odd. Many were odd when
they were children; much like
some of their relatives who al
ways suffered with psychotic
symptoms.
Years ago, my good friend,
Dr. A. E. Bennett, an eminent
psychiatrist of Berkeley,
Calif., showed that a consider
able percentage of persons
who, nowadays, have their ab
domen explored surgically
never did need the operation;
what they needed was help
from a psychiatrist. A while
ago when I reviewed the rec
ords of 674 of my more ec
centric patients, seen in the
last 10 years, I found that one
in nine had received an oper
ation that hadn't done any
good because it was ill-advised.
It wasn't what was
needed.
Average Teacher
As I have been pointing out
for many years, even the aver
age teacher of medicine does
not recognize a decidedly
schizophrenic or maniac-depressive
patient. Why doesn't

medical school, he was not
shown such patients; what he
saw were thousands of per
sons in a charity hospital
persons suffering usually from
the end results of diseases of
the heart and arteries and kid
neys; from arthritis, cancer,
tuberculosis, pneumonia and
other severe infections.
No one ever taught the pro
fessor to recognize a psychotic
patient at a glance, or from
the way in which he tells his
story, or from the nature of
his story. For instance, until
perhaps 15 years ago, if a man
told me that blood was trick
ling through his chest, or pus
was running from his prostate
gland up into his brain, or
worms wer crawling around
under his skin, I did not real
ize right off that he was prob
ably a schizophronic, much in
need of a psychiatrist. I did
not know what the common
symptoms of mild schizo
phrenia " or manic-depressive
psychosis are, and I did not
know them until I had re
viewed the records of close to
a thousand patients I had seen
with these mild mental trou
bles. Let us remember that
these patients I saw had come
to me as an internist, for diag
nosis; they had not gone to a
psychiatrist, because they did
not know they needed one.
What we- need terribly in
medicine today is a better
training of our medical stu
dents in the quick recognition
of the mildly psychotic pa
tient whose main symptom is
a pain in his or her abdomen.
Our medical students don't
want such training because
they labor under the delusion
that they will never see men
tally disturbed patients when
they get out into practice.
