The young physician who
decides to be an internist usu
ally will spend his post-intern
year as a "resident" in a big
hospital, working on the
"medical" side of the institu
tion. After this, he will spend
probably three years work
ing in a graduate medical
school or in
a big clinic
learning the
fine arf of di

and unusual
and puzzling
diseases; also,

After these three years the
doctor will be entitled to take
his National Board examina
tion and to become a "diplo
mate." Then, and only then,
after 12 long years of study,
can he call himself a. special
ist. But, even after this, he
may decide to specialize more
narrowly, perhaps in cardio
logy (heart disease) or gastro
enterology (diseases of the
stomach and intestines), and
again, he may have to take
an examination and receive
another diploma.
After that, if the man is
going to be a fine consultant
physician, he must keep on
studying, reading each month
at least a half-dozen medical
journals, and perhaps teach
ing in his spare time in a hos
pital or clinic or medical
school. Many an able special
ist, for much or all of his life,
gives gratis a part of his day
to working for the poor in the
free clinic of some university
or hospital. Few people real
ize when their child's harelip
or defective hip joint is beau
tifully repaired by a great
surgeon, or the child's very
puzzling fever is diagnosed
and cured by an eminent pedi
atrician, how much of the
man's experience and skill
were acquired while treating
hundreds of children in a free
of part-pay clinic. .
Specializing Within Speciality
Many a specialist, and es
pecially the man who works
in a big clinic to which come
each year thousands of pa
tients, specializes within a
specialty. For instance, at the
Mayo Clinic, some men spe
cialize in diseases of the
mouth, while others are par
ticularly interested in diseas
es of the gullet, or ulcers of
the stomach and duodenum
(bowel just outside the stom
ach.) Other men study diseas-

Today, there is so much to
be known about just one di
vision of medicine such as
gastroenterology that, I
who have studied this special
ty all my days and have writ
ten several big books on it, do
not feel competent to treat
acute abdominal diseases in
a hospital; because, for 40
years, I have not handled pa
tients with such troubles.
I would hesitate, also, to
make the diagnosis in a puz
zling case of jaundice or pos
sible pancreatitis because
these diseases I seldom see.
My particular field has been
that of the diagnosis of cronic
digestive troubles and espe
cially those of nervous or
psychic origin.
Many persons criticize us
doctors for specializing so
much, but we cannot help
ourselves. I cannot possibly
keep up-to-date on what I
must know about the diagno
ing of people with very puz
zling and bizarre abdominal
discomforts, and at the same
time know all about passing
a lighted tube down into the
stomach or about spearing a
sample of tissue out of a di
seased liver. I cannot know
much about the many diseases
of the rectum.
One of the delightful fea
tures of working in a large
clinic is that every one of the
physicians can so easily call
a colleague into his room and
say, "Listen to this man's
story and look at him, and
tell me if, in all your enor
mous experience, you ever
saw a case like this."
Today, in a big clinic one
can find internists whose
main interest is in diseases of
the heart, the stomach and
bowel, the glands of internal
secretion, the blood, the lungs,
the liver, arteries and veins
and high blood pressure, or
tropical diseases or diseases
of nutrition, or diseases of
nervous origin.

middle-aged or elderly man
walking with a limp and per
haps using a cane. He may be
a bit overweight, and he com
plains of pain and stiffness in

by rest. .Sometimes the ache
becomes so bad that it inter
feres with sleep. It is aggra
vated by damp weather and
cold. Often a laboring man.

will show that it is so gnarled
and deformed by bony disease
that one cannot hope to cure
the man with any medical
treatment. I used to say that
these unhappy people could
be cured only by getting a
new joint from the factory!
Of late, surgeons have been
cutting in, taking out the old
joint, and putting in a new
one made of a metal or a
plastic. In some cases, the re
sults have been good; -in
others, the pain has not been
relieved. It is to be hoped that
during the next few years the
technic for making the new
joints will be greatly perfec
ted. Dr: Alvarez hopes his read
