Endocrinology is a special
ty practiced by physicians
who treat people who suffer
from too much or too little
of one or more of the several
hormones produced by their
glands of internal secretion.
The doctors treat by giving
thyroid substance, or drugs
made from the ovary, the tes
ticle, the adrenal, the para
thyroid or the pituitary gland.
Every endocrinologist has in
his files hundreds of pictures
of children and adults, de
formed, misshapen, fat, pudgy,
asexual, coarse, hairy, or
much too short or much too
tall because of something that
once went wrong with one of
their glands.
If the diagnosis Is made
early enough, some of these
patients can be helped tre
mendously. I will never for
get a teenage girl I saw one
day with Cushing's syndrome,
due to a tumor in one of her
adrenal - glands. She looked
coarse, with much hair on her
face and a big abdomen with
big striations on it. A few
weeks after a surgeon had re
moved the tumor, I was as
tonished when the girl came
in, slimmed down, and de
cidedly good-looking. It was
a miracle.
Sometimes Miracles
A miracle can sometimes
be performed also for a lad
whose testicles never develop
ed. He is given weekly doses
of an androgen or male hor
mone. Curiously, the two com
monest diseases that are due
to a disturbance in a ductless
gland or "gland of internal
secretion" are seldom treat
ed by an endocrinologist.
One is diabetes, which is
due usually to a destruction of
certain cells in what are call
ed the islets of Langerhans of
the pancreas. They are little
clumps of special cells imbed
ded in the substance of the
big digestive gland which lies
back of the stomach. Certain
of these "islet cells" have to
do with the body's handling
of sugar. Diabetics are gener
ally taken care of by internists
who specialize in the treat
ment of the disease. Diabetic
children are often handled by
pediatricians. Diseases of the
thyroid gland used to be treat
ed mainly by surgeons and
internists. Today, many of the
patients are taken care of by
radiologists who use radio
active isotopes. . . -
With too little thyroid ac
tivity the patient usually a
woman will feel cold and
dull and sleepy. She may go
to sleep in her chair. She gen
erally gains weight; her face
may get pudgy and her skin
and hair will be dry. Usually
she loses sexual interest.
When given thyroid substance,
she is likely to wake up and
to feel immensely better. She
may lose weight, and she may
become bright and attractive
again.
Most people who are over
weight keep hoping that their
trouble will be found to be
due to purely hypothyroidism
or some other glandular dis
ease, but it practically never
is. Tests made on stout peo
ple show that their basal me
tabolic rate is almost always
normal. They get fat because
they have too good an ap
petite. Each day they eat more
more food than they actually
need, and some of this is
stored as fat.
Persons with hyperthyroid
ism or a toxic goiter are too
active and wide-awake. They
cannot sleep well; they are
restless; they blink their eyes
frequently, they perspire too
much; they feel too warm;
they may eat much, but they
are likely to lose weight, and
their heart races. Such per
sons are now cured with
either drugs or a removal oi
their goiter.
Some Not Curable
Many a patient comes in
to say, "My physician thinks
my troubles are glandular in
origin, and for six months
he has been trying out shots
of one hormone after another.
So far, none has helped. Can
you think of another one to
try?" I go at the problem by
first listing all of the pa
tient's prominent symptoms
to see if they fit into any of
the known syndromes (groups
of symptoms) due to too little
or too much secretion com
ing from any one of the sev
eral glands of internal secre
tion. In most cases, I find that
the person's troubles cannot
be fitted into any one of these
known syndromes, and hence
I 'much doubt if she has any
endocrine imbalance. I know
some physicianos who will
say, "She has a sub-clinical
disease,'; which means she has
the disease in such a mild
form that it has no symptoms.
That, of course, means that
the doctor is just guessing,
and I cannot argue with him.
I recently read an article
by a very fine endocrinologist
who said that any person

whose disease remains puz
zling after many examina
tions, is likely to get to hoping
that perhaps he could be
cured by "shots" of some hor
mone. But as the expert said,
"most of them must be disap
pointed. Even very hairy
women can seldom get help
from an endocrinologist, un
less their hairiness is due to
an easily recognized Cushing
syndrome." I admire this
man's wisdom and frankness
in saying that in his experi
enc the psychic disturbances
of homosexual persons and
transexuals (who want to
have their official sex chang
ed) are not curable by any
drug he has at his disposal.
Dr. Alvarez hopes his read
ers will understand that it
would be imossible for him
to answer requests for infor
mation or to attempt to diag
nose by mail.
