ered with the x-ray that
they have an

want to know
whether they
should prompt

on. My questions, of course
immediately are: Are you real
ly ill? Are you suffering from
the symptoms of an ulcer? Do
vou fail to get good relief
from diet and medicine? Are
vou sure you have an active
ulcer?
It seems to me utterly
stupid to be operated on sim
ply because there is a slight
deformity of the duodenal
wall. A hundred times I have
said to the patient when he
came in, "Have you any pain
now?" And he has said, "No,
I'm perfectly comfortable.
just happened to get an ex
amination as an annual check
up." Then I asked, "When did
you last have pain that came
when vou were hungry and
went away as soon as you
ate?" And the man, perhaps
aged 45 or 50, said, "Oh, that
was when I was about 22
For about two months I had
rjain like that, and then it
went away and never came
back."
Obviously, then, all that the
x-rav man had seen was the
scar of an old ulcer. Once a
man has a deep ulcer in his
duodenum, he generally has
a puckering scar for the rest
of his days. The only way of
telling whether he has an ac
tive ulcer is to ask if he has
a pain tnat comes, usually,
about 11:30 in the morning,
5 in the afternoon, and per
haps midnight. Women have
ulcer less often than men, and
when they have it, their story
is not so typical. For this rea
son, it is much harder to make
the diagnosis in a woman than
a in a man.
Food Between Meals
In my long experience,
most persons with an ulcer
are able to get by well enough
by taking food between meals,
and perhaps some acid-com
batting medicine. Many a per
son with ulcer has only an
occasional short spell of pain,
and then he goes two or three
years without trouble. Fortu
nately, in most cases, the dis
ease is mild. Only rarely do
see a man with a very se
vere ulcer with almost un
controllable pain.
Often he can lose his pain
immediately if he will only
close his office and go on
vacation. As I often say
to these men, "Why have an
operation which may or may
not cure you, and may leave
you worse than you were be
fore, when you can get well
so easily by cutting down
on the strain of life?"
I remember a man who was
running a large and very dif
ficult business in which he
had great trouble with gang
sters, who were trying to mus
cle in. He had an ulcer so
bad that at night, instead of
sleeping, he walked the floor
in pain. When he came to
Rochester to see me, I put him
into the hospital and said to
the resident, "Tonight this
man will probably need
heavy doses of some sedative;
I expect he'll give you a good
deal of trouble." Next morn
ing, when I dropped in to
see the fellow, the resident
said, "Why were you so anx
ious about mat man; ne
slept like a log all night!"
Actually, during the next two
weeks, while the man was
in Rochester, he had no pain
at all. About all I did in the
way of treatment was to get
him to turn over most of his
business to a husky partner,
and years later, when I ran
into him again, he told me
he had been perfectly well.
Couldn't Stand Gaff
I'll never forget the fine-
looking man whom I found
running a bus-stop station in
the mountains out in Wash
ington state. As he put gaso
line m my car, l cnattea
with him and finally said,

ulcer. I came out here, snd
ever since I have been per
fectly well."
The other day I saw a man
from Venezuela who said he
had been running three big
businesses. He said that for
weeks he had been walking
the floor at night with the
pain of a terrible ulcer. But
he said, "Doctor, this is curi
ous; the day I left my office
and got on the plane to go
north, I was well. I have been
eating all sorts of indigestible
food to see if I could get my
pain back,, but I can't do it,
and I am sleeping like a top
all night."
Boss Hard Man

with an intractable ulcer
that had not responded to any
treatment given him by three
or four good physicians. I
simply said, "What is your
job? Do you like it and is it
easy?" He said, "I am in ad
vertising, and my boss is a
very hard man to work with.
His favorite stunt is to say
to me on Friday afternoon,
'Here are notes on a new ac
count that we have just got
ten; come to me Monday with
a whole lot of ideas for a
campaign." I . had this man
throw up his job and go on
a vacation, and with this, he
immediately was well.
Obviously, in these cases,
we know that when the pain
is gone the ulcer cannot be
healed because it takes at
