anism which is lodged within
one of the bones of the skull.
In such case the doctor
speaks of a conduction type of
deafness! When the trouble is
in the inner ear or in the
nervous pathway to the brain,
the perceiving mechanism is
impaired, and the patient has
a perceptive or nerve-type of
deafness. Usually, there is
mighty little that can be done
for persons with a nerve-type
of deafness.
A common type of deafness
is due to otosclerosis, or a
hardening of the bone around
the inner ear. This involves
the region of the little window
which connects the middle ear
and the inner ear. Into this
window is fitted a tiny bone
called the stapes, or stirrup.
One form of deafness comes
when the stapes becomes so
firmly stuck to the window
that it cannot vibrate back
and fourth as it normallly
does.
Otosclerosis is hereditary,
and a little of it can be present
at birth. It tends to get worse
between the ages of 20 and
40. Fortunately, few persons
with this disease go on to
total deafness. Ear noises are
often present. Unfortunately,
there is no medical treatment
that is of any value.
Some 20 years ago, Dr. Ju
lius Lempert, of New York
City, developed the fenestra
tion operation in which a tiny
window is made between the
middle and the inner ear. With
this, thousands of people,
formerly deaf, are now hear
ing again. More recently, Dr.
Samuel Rosen, of New York
City, devised an operation for
a loosening lip of the stapes.
Often, today, this operation is
tried first. If it works, well
and good; it helps about six
out of 10 patients. If it does
not work well enough, or if
it does not stay working well,
the fenestaration operation
can be tried.
Pain After Shingles
I get so many appeals from
old people, asking me what
on'earth they can do for the
miserable neuralgia that has
followed an attack of shingles,
or of herpetic blisters on their
forehead, or around one eye,
that I keep writing about it
Modern Medicine contains an
excellent report by Dr. Ken
neth H. Abbott, of Columbus.
He starts out by admitting
that the disease is a bad one,
and one that often wrecks the
nervous system of the victim.
He admits that the treatments
available rarely help or help
for long.
Temporary alleviation of
pain may follow nerve blocks
with procaine. Dr. Abbott con
firms the statements of -some
say that mechanical vibration,
say that mechanical virbation,
and particularly, pressure
against a hard surface, or tight
binding and bandaging can
give some relief. For instance,
Dr. Abbott tells of a woman
who always got help by press
ing her painful forehead
against a cold window pane.

have read, that surgical mea
sures are usually disappoint
ing. One can cut nerve-roots;
one can take out nerve-ganglions;
and one can lift up and
cut all the nerves under the
area in which the pain is felt,
but the results are usually dis
appointing. Dr. Abbott says
that the cutting of pain-carrying
tracts in the spinal cord
has also been disappointing.
The doctor concludes by say
ing that we just do not have
any good treatment for the
pain.
Dr. Alvarez hopes his read
ers will understand that it
would be impossible for him to
attempt 'to answer requests for
information or to attempt to
diagnose by mail.
