is less vibration, heat, noise, and pressure.
The patient feels less tension, and the
doctor is not so tired. Shorter visits also
enable the dentist to serve more people.
This is especially important today, when
our population growth is running far ahead
of the number of dental graduates. .
In 1952, the National Bureau of Standards"
developed a . water-driven turbine hand
piece that operated at 45,000 rpm, compared
with 2,500 to 6,000 rpm for the old tool.
This work was continued, with somewhat
different approaches, by a number of
agencies, as well as by private dentists,
until Dr. Roderick A. McEwen of Atlanta
was able to describe the latest device to
the American Dental Association's 1956
meeting as one used "like a paint brush
with a feather touch."
"Often the patient cannot locate the site
of the treatment," Dr. McEwen said, and
he added that the absence of nerve inflam
mation after preparing cavities was reason
enough to accept the new instrument.
In addition to greater speed, efficiency
has been further enhanced by carbide and
diamond tips instead of the old steel burrs,
and a water-air spray for cooling . and
cleaning the tooth surface enables the
dentist to work without interruption.
An inventor-dentist, Dr. Robert B.
. Black, developed the remarkable
airbrasive machine, which prepares
cavities with an abrasive stream shot from
a tiny nozzle at terrific speed. Though this
machine has been accepted by many
dentists, Dr. Black reported in a ten-year
summary in 1954 that it was often not
suitable for teeth with large decayed areas
and restorations. Other dentists have found
the equipment is bulky in small offices.
A device that gives a feeling of "magic"
to dental work is the ultrasonic instrument
adapted from an industrial drill in 1954. It
operates by vibrations too rapid and high
pitched to hear. The vibrations act on an
abrasive to cut tooth structure.
Here, it appears, is something that might
lead to "painless" dentistry, the answer to
the dreams of both patient and dentist. Dr.
Carl R. Oman of Columbia University
reported that the instrument produced
practically no vibration and patients felt no
discomfort. Dr. Oman said it cut enamel
smoothly, but it was ineffective on decayed
dentin. Nevertheless, it could be used for
80 percent of the work on cavities.
But two studies since 1954 seem to
indicate that the ultrasonic instrument
hasn't fulfilled its early promise. The results
are far from conclusive, and doctors feel
there is still much to be learned about
ultrasonic equipment. As Capt. Arne G.
Nielsen of the U. S. Navy Dental School
said, "The attempt to determine the com
plete safety of the ultrasonic process in
continued studies appears justified." And
in his Illinois report, Dr. Jones concluded
that "ultrasonics may be something that
dentistry will use in the future."
So you see, your dentist would like
nothing better than to throw away that
"drill" that's been the butt of so many
jokes. And thanks to dental research, may
be he'll be able to some day.
