When a Child Swallows a Marble

the other day choked on a
marble wonders what she should
have done to
help him. All
she could think
of was to turn
him upside
down and
thump his
back. Fortun

marble, but an-

tative information, I turned to
Dr. Paul H. Hollinger. of Chi
cago, who is an expert in getting
things out of children's air pass
ages and gullet. He said. "Al
most all foreign bodies, whether
in the larynx (voice box), the
bronchus (one of the air tubes
in the lungs), or the esophagus
(gullet) will cause severe cough
ing, choking and gagging. Slap
ping the child on the back to
dislodge the foreign body is po
tentially dangerous because if
the object is in the bronchus
it is possible for it to be dis
lodged and become stuck in the
larynx." Then only a quickly
made tracheotomy would save
the child. In this operation a cut
is made into the windpipe just
below the thyroid gland, below
the Adam's apple.
Dr. Hollinger believes that it
is best to keep the child as quiet
as possible and rush him to the
hospital. He says that marbles
and jacks are particularly
dangerous even wrhen they are
in the gullet. Bringing the child's
head forward and lifting the chin
at the same time, being careful
not to tilt the head far back
ward, will improve the airway ;
and that can help. Sometimes a j
large lump of food in the gullet
will press on the windpipe and
then only quick expert help can
save the child's life.
Keep Objects Away
One of the important things
that parents of small children
can do is to try to keep from
them objects which they can get
into their windpipe. Often, I've
had the creeps as I have watched !

peanuts, an almond, a bean, or
an olive with a pit in it.
One of the worst things that
women do is to give their baby
a button basket to play with; j
this is just asking for terrible 1
trouble. It's a marvel to me that
the infants who are constantly
putting things in their mouths
ever survive! The infant, if left
alone with an olive, might get by
all right, but when his brother j
hits him or tumbles onto mm
down the windpipe it can go. I
People should know that in !
every large medical center there ,
are men who are expert in look-;
ing into the windpipe or the j
gullet with a lighted tube. They
are expert in fishing out foreign
bodies. They have to have spe
cial forceps with which to get
hold of rounded bodies, like
peanuts or marbles. They have
a special apparatus to close
safety-pins.
Commonly, an infant will swal
low an open safety-pin and this
must be closed before it can be
gotten out. When something is
stuck in the windpipe or gullet
the quicker an expert gets at it
the better. After .a while, the
lining of the tube becomes so
swollen that it is much harder to j
get at the thing and to get hold j
of it. If a child is strangling for :
air, a doctor at the nearest hos
pital can cut into the windpipe
and save life.
Some of The Less Common
Causes of Severe Backache
SLiny a person with an acute
backache asks me what is wrong, i
and I cannot tell from a distance.
I might not be able to tell if
I could examine him with care.
Many a person with a bad back
wonders why x-ray films of the
spine show no sign of disease. ,
Also, an expert orthopedist, when
called in, may examine the back
and he may see nothing wrong.
Then, if, as often happens, after
a while the backache goes
with or without some manipula
tion or physiotherapy the
chances are it was an episode
in a life-long tendency to arth
ritis or fibrositfs.
But, says the patient, "Why
did it come upon me at that par-:
ticular time?" I suspect that ,
often an acute backache, if not i
brought on by lifting or getting
a crick, is due to an infection
with a virus. I know I once got
a terrible and paralyzing attack
of lumbago due to the polio-like i
Coxsackie virus. This produces
a disease often called "the
Devil's grip," and it surely feels
like that! In some cases, the virus
affects the pleura which lines
the chest cavity and it may in
volve the sac in which the heart
lies.
Smallpox Appears
Years ago in Southern Cali
fornia, a man got a very severe
backache, so bad that he entered
a little hospital run by an irregu
lar practitioner. After a few days
of the terrible backache, he
broke out with a malignant type
of smallpox with black pustules
all over his body. As a result,
if I remember correctly, the prac
titioner, the sanatorium cook,
and a number of people around
the place came down with a se
vere form of smallpox and sev
eral died.
A few months ago I was re
minded of this case when in
came a previously perfectly
healthy young woman with a
severe low backache the first
she had ever had. The pain was
so bad that for several nights
she could hardly sleep. Aspirin
could not touch it. What was the
cause? Apparently, an unusually
severe reaction to the vaccina
tion against smallpox which she
had had a week before the pain
came. The "storm" soon quieted
down and the woman has had no
trouble since.
Dr. Alvarez hopes his readers
will understand that it would be
impossible for him to answer re
quests for information or to at
tempt to diagnose by mail.
