The Troubles of a Doctor Who
Sees 50 Patients a Day
The other day a man scolded
me for having mentioned the
fact that some unfortunate phy-

these doctors
take on this
terrible burden
of work be
cause they are
heartless mon
ey g r a b b ers.
my scolder was
not well informed. The doctors
who see so many patients a day
are usually not the money-grabbers.
I have known quite a few
physicians of the old school who
saw from 50 to 100 patients a
day, and charged each one only
a dollar or two for the visit.
Some of them "threw in" a bot
tle of medicine!
Obviously, such physicians
cannot either examine a patient
thoroughly, or do much labora
tory work or x-ray work. They
can only make a quick guess as
to what the trouble is. I could
not bear to practice in such a
way. To me, a life like that
would be an awful treadmill.
And yet, I have respected thor
oughly many of the doctors 1
have known who have done this
type of work. They felt that
with their clientele of poor peo
ple, it was either this type of
practice or none at all.
The man who scolded me said
that the cure for such over-bur
dening of doctors is socialized
medicine. Actually, the experi
ence of countries like England
shows that the cure is not in the
socializing of medicine. Social
ized medicine, with its small
charges per patient, is likely to
compel a man to see from' 50
to 100 patients a day in order
to get a living for his wife and
children. And this compels him
to practice a poor type of medi
cine. Well-Educated Man
Once, when I was in a foreign
country in which most people
do not have to" pay anythinS
when they go to the doctor, I
watched a physician as he took
care of perhaps 30 patients in a
couple of hours. And I saw this
particular type of medical prac
tice at its best because my friend
was a well-educated man, a
kindly man, and an idealistic
one one who loved medicine
and hated the poor type of work
he had to do. He hated it so much
that he was seriously thinking
of giving up medicine and going
into business.
As my friend took care of
those 30 people he never even
greeted them as they came in or
said goodbye as they left. He
did not look at them and he
seldom asked any questions. In
each case, a very short history
had been taken for him by a
nurse. One can imagine how in-

of a diagnosis. If the patient
needed surgical work, he was
sent to a hospital; and if he
needed medicine, the doctor
wrote on a slip of paper "No.
66" or "No. 85."
If the nurse remarked that
he had already had medicine
number 66, without improve
ment, the doctor would say, per
haps, "No. 94." If the nurse said,
"This fellow has been here four
times and still has his cough,"
the doctor might initiial an order
to send the fellow into ahospital
for an x-ray study of his lungs.
If I had a son contemplating
going into medicine in that par
ticular country, I would beg him
to try something else. Socialized
medicine of that type is no occu
pation for anyone who hates to
be ashamed always of the work
he is doing.
Some may . ask, "But why
didn't he stop and do good
work?" Because he had a wife
and some children to educate,
and being paid only a few cents
a visit, he had to see many pa
tients a day.
Help in Educating
Spastic Children
I have just been reading an
excellent book by Dr. F. Eleanor
Schonell (Philosophical Library)
on the education of spastic chil
dren. Many mothers of spastic
children wish very much that
thev could find some informa
tion in regard to them, and espe
cially in regard to teaching them
all they have capacity for learn
ing. Dr. Schonell shows that
while some spastic children are
mentally retarded, many are of
average intelligence, and some
are bright. The "book tells of the
many things that some silastic
children can be taught, and how
they can be taught. Some of the
book is a bit technical, but all

it with great profit. If any of
them want to get the book, they
should write to the publishers
not to me.

ers of spastic children should
read is Christy Brown's auto
biography "Mv Left Fnnt '
This very bright Irish lad, who
once could not walk or talk or
even feed himself, is now im
menselv better and has written
a delightful book (Simon and
acnuster) full of understanding
of the many problems of a spas
tic cniia.
Dr. Alvarez hopes his readers
will understand that it would
be impossible for him to answer
