Electxoihock Tratmnt
Many persons want to know
how electroshock treatments are
given, will they hurt at the time,
will they do
harm, and can
they do good?
If so, in what
type of mental
illness do they
do good? At a
recent big con
vention some
psychiatri s t s
were accused
Mailrl Alvarri Oy Uieir iei-
lows of giving electroshock
treatments to almost all of their
patients, just on general princi
ples. Many psychiatrists criticize
this practice, and said that, in
their experience, about the only
type of patient who is benefited
by electroshocks is the one who
is depressed and melancholic
and "blue." They doubted if a
psychiatrist was fair to his pati
ent if he gave the shock treat
ments without a preliminary
study, and they did not think
much of a psychiatrist who
never talked to patients, but
just gave them electroshocks.
In cases of mental depression
I have seen miracles performed
with the help of mild electro
shocks. Persons who had been
prostrated for a year went right
back to work. In other cases in
which the person was not pri
marily depressed, . neither the
family nor I could see that any
good was done. A few of the
patients who had had many treat
ments complained for a while
of some failure of memory.
Strong Current Unnecessary
There are a number of types
of apparatus used in the treat
ment, and a number of types
of current. In the old days when
strong currents were used and a
big convulsion was produced,
bones were sometimes broken.
Today, with the use of much
weaker currents and the avoid
ance of a convulsion, I seldom
see a broken bone. I doubt if this
is necessary because I have seen
such good results from treat
ments with gentle currents which
hardly convulse the patient at
all. Some psychiatrists just
touch the button of the appa
ratus for a moment, the patient
instantly becomes unconscious;
he stiffens out, and this is all.
In half an hour or so he may
get up and go home with his
relatives.
Some men who know that the
patient cannot feel anything, do
not give any preliminary medica
tion, while others, knowing that
many persons are very apprehen
sive and afraid, give first a little
anesthesia. Many psychiatrists
put the patient in the hospital
over-night, while others have the
patient come into the hospital
only to stay for half an hour in
the treatment room. A few give
the treatment in their offices,
which greatly lessens the expense.
Some men will give three or
four treatments, and then if the
patient is no better, they quit.
Other men will go on and give
20 or 30 treatments, even if the
patient shows no sign of im
provement. This, I suspect, is
mistake.
Many patients have a little
impairment of memory for a
while after the treatment, but
especially when the person gets
only three or four treatments,
this difficulty with memory is
not likely to be distressing, and
it is likely soon to clear up and
go away.
If I had a depression which
distressed me greatly and made
it impossible for me to work,
and if this depression didn't
soon clear away, I would want
to have some electroshock treat
ments. Some opponents of this
treatment might say, "But you
may be back again in eight or
ten months, depressed." and my
answer would be, "All right, I
will then take some more shock
treatments and try to Ret well
again."
Exclusive Use Distressing
As some psychiatrists pointed
out at the last two big meetings
of the psychiatric society, it is
distressing to them that some
of their brethren use electro
shocks excessively. They use
them on practically every pa
tient who comes in the door.
They don't bother to talk to the
man: they just say, "Get up on
the table there and I'll give you
an electroshock." Many psychi
atrists think that this is wrong;
and they think it is wrong, also,
when a psychiatrist spends a
year or two trying to analyze a
person who has a depression.
They think it useless to treat a
depressed patient with psycho
therapy. They believe that the
old psychiatrist were right when
they said that you cannot talk a
depressed person out of a mel
ancholia. It is a disease like a
broken arm which mends only
with time. There is no need of
exhorting the man to get well.
He is suffering terribly and
would love to get well.
Any student of medical history
or any wise old doctor always
looks askance at a brother physi
cian who treats all of his patients
the same way. One can be al
most certain that such a man is
making many mistakes, and he
certainly isn't using the brains
the Good Lord gave him. Any
physician who treats all his pa
tients with cortisone or with pen
icillin or with shocks, or by
twisting his back must be mak
ing many mistakes. Why? Be
cause there are a few hundred
diseases with different causes
and hence different treatments.
Dr Alvarez hopes his readers
will understand that if would be
impossible for him to answer re
quests for information or to at
tempt to diagnose by mail.
