The Need For Knowing
Findings At An Operation
A woman was just in with a
story which suggests a scatter
ing of cancer in her abdomen.
I learned that
a year ago she
had an opera
tion perform
ed because of
some discom
fort in her pel

course, I asked

the time she had been unable to
luarn an vthine either from the
surgeon or his assistant or the
hospital. I sent for me nusoana,
an intelligent business executive,
and asked him if he had been
told, and he said, "No," he haa
been unable to find out what had

the surgeon who performed the
operation has retired; and I can
not reach him for questioning,
hence I cannot get the informa
tion which I so need if I am to
make the correct diagnosis.
A mv old chief. Dr. William
J. Mayo, once said to me, "How
wonderful it will be some aay
whon everv time a surgeon op
erates he will give the patient or
her husband or some responsible
person a copy of the report
uhirh in most hosDitals. is dic
tated as soon as the operation is
finished." The surgeon snouia
give, also, a copy of the report of
the pathologist (trie man wno
examined microscopically the
tumor or the material removed,
and decided whether or not it
was cancerous.)
Such action on the part of sur
geons would often be so helpful
to the patient, and so helpful to
the doctor who, later, may have
to guess whether the person's
pain and loss of weight are due
to the return of a cancer.
Would Conflict Wilh Idea
I know that the behavior .that
I here advocate would conflict
with the commonly held medical
idea that the patient should nev
er be told the truth about the
presence of cancer. However, as
I have said many times before in
this column, after 50 years of
watching the results of both ly
ing and truth-telling (to patients)
I am strongly in favor of telling
the truth.
In my exeprience, lying sel
dom works well. Usually the doc
tor is a poor liar and the patient
sees through him. Commonly, al
so, the relative who is told the
truth, soon gives it away either
inadvertently or because he or
she cannot hold a secret. One of
the sad features about lying is
that when a man comes in with,
let us say, a harmless benign
tumor, or a harmless pain in his
chest, and after an examination
I tell him I haven't found any
sign of cancer or heart disease,
he doesn't believe me; and there
is no way in which I can reas
sure him.
Why isn't there? Because, as
he says, after watching doctors
lie to his dying father and later
his dying mother, he feels sure I

Today I received a letter from
a woman who says that since
learning from me that she had a
cancer removed at the time of
her operation, she has stopped
worrying. She has faced the fu
ture bravely; she has put her
house in order, and she is not
afraid. It was uncertainty that
had bothered her.
Should An Old Person
Be Operated On?
Several times in this column
I have remarked "hat if I had
healthy and wide-awake and
spry old relative who was 80 or
8a years of age, and who needed
an operation for a big rupture or
for cataracts, or for some painful
disease somewhere, 1 would not
refuse my permission. I would
assume that the man (or woman)
was competent to decide whether
he cared to take the risk of dy
ing, and I would feel that he still
retained all of his original legal
rights. Why should I act as if he
were a child and I, his parent?
According to the Chinese, be
cause of his age he should have
more wisdom than I possess.
Also, I would not know how
little or how much he cared to
go on living. Many old people
are not particularly desirous to
hang onto life for a few more
years. Many feel they have lived
long enough. They say, "I would
not mind having a few more
years, but I want them only if
they are to be comfortable and
happy years. If extra months or
years are to bring me only suf
fering and the wastage of money
that is much needed for the care
of my wife or the education of
my grandchildren, I do not want
those months or years." Many
say, "If this operation should
cause me to lose my life, that is
all right; I won't mind at all."
Often I have been distressed
when I have heard a man say,
"When, during the last 15 years
of her life, my mother sat around
practically blind with cataracts
