GALLBLADDER OPERATION

Any number of nersons ask
why they are just as miserable
after their gal'bladder operation
as they were
before. I can
not answer
them without
asking them a
number of
questions, and
.even then I
might be puz
zled. If, before
the operation,
a woman had
rx
touibiones auu was suiiering
from colics needing morphine,
then we can be almost certain
that her symptoms were due to
a diseased gallbladder, and her
operation was needed.
If, now, she is still suffering
from occasional spells of the old
pain in the abdomen just below
her ribs on the right, and es
pecially if these spells are asso
ciated with vomiting and brief
fever, I would suspect that at
the operation some stones were
missed and left behind in the
duct that runs from the liver to
the bowel just below the stom
ach. One cannot blame the sur
geon for this because such stones
are often very hard t8 detect. In
a case of this type the patient
may be forced to accept another
operation. Since it may be a very
difficult one, it should be per
formed by an unusually able
surgeon.
In my experience, the persist
ence of a lot of nervous dis
tresses after a gallbladder opera
tion is due to the fact that the
gallbladder never was responsi
ble for the symptoms: They were
all coming out of a nervous or
troubled brain.
In many case today an X-ray
man, trying to be helpful, re
ports that the gallbladder emp
tied a Jittle slowly, or it was
slightly deformed (usually since
birth) or it looked as if there
were adhesions about it. Then
the surgeon, also wanting to be
helpful, removes what turns out
on examination to be a normal .
gallbladder. Naturally, this does
not do any good.
In most cases, Gwhat happened
was that there were some stones
in the gallbladder but they were
not produ3ing any of the symp
toms complained of. They were
about as harmful to the woman
as if they were out in her hand-
bag. She never had a colic and
she had a good digestion without
any flatulence. Naturally, then,
the removal of her gallbladder
could not possibly have done her
any good.
Why did the woman's physi
cians make the mistake? Because
in our medical schools there is
no one to teach the students
one of the most important
courses that is needed, and this
would be a course on the art of
disregarding those findings that
have nothing to do with pro
ducing an individual's symptoms.
Because, when I was in college,
I never had such a course, I can
remember 40 years ago promis
ing a woman with gallstones
that if she would let a surgeon
remove them she would be rid of
her migrainous sick headaches.
She had her operation; her
headaches became worse, and
then and there I learned my

with shame another case in
which I found that an elderly
woman w-ith a constant "misery"
in her whole abdomen had a
fibroic1 tumor of her uterus.
When I got a surgeon to take it
out she was no better. Then I
did what no teacher had ever
taught me to do; I talked to the
family and found that my poor
patient was almost insane. She
spent each night walking through
the house, turning on all the
lights, wringing her hands and
weaping in an agony of restless
ness. She belonged in a mental
hospital.
I remember sadly another case
of a handsome man of 50 who
spent much of every night belch;
ing, walking the flooor, taking
soda and rubbing his abdomen.
When the x-ray man said he had
seen an ulcer, I, in my ignorance,
had the fellow operated on. All
the surgeon found was the scar
of an ulcer the man had had
30 years before! I did riot know
