AFTER THE ULCER STOPS
A bleeding peptic ulcer may
be located either in the stomach
or in the adjacent bowel very
rarely in both.
The high rate
of develop
ment of cancer
in ulcer con
fined purely to
the stomach,
Is such that
the present
consensus of
informed opin
ion is that such
cases, bleeding
or not, should
be operated
upon without

delay. An in
strument has been devised which
can be passed into the stomach,
through the mouth. By this
means an ulcer of the stomach
may be viewed directly. Unfor
tunately, it can not always be
determined by viewing the ulcer
in this manner, whether or not
it is cancerous.
Ulcer of the duodenum duo
denal ulcer is often included
under the term "stomach ulcer."
Actually, of course, It is not an
ulcer of the stomach. Duodenal
ulcer very rarely is cancerous.
Thousands of persons have suf
fered from duodenal ulcer for
years on end, finally dying of
some other disease.
When ulcer of the duodenum
produces a' scar which contracts
the bowel obstructing it to
some degree surgery is neces
sary for permanent relief, de
pending, of course, upon the de
gree of the obstruction. Bleeding
or hemorrhage from a duodenal
ulcer may occur when no ob
struction is involved. Bleeding
mav be slight or it might be
furious and terminate in death.
As a general rule, hemorrhage
from duodenal ulcer is treated
non-surgically. If the hemor
rhage is persistant and uncon
trollable, surgical interference
becomes necessary. This inter
ference imposes upon the victim
a life-saving procedure but one
not conducted under ideal cir
cumstances as far as surgical
treatment of the ulcer is con
cerned. Inasmuch as most pa
tients with hemorrhage survive
without surgery, the question of
future management must be con
sidered. It being necessary to take a
stand somewhere in the matter,
many surgeons subscribe to the
view that a person who has suf
fered three consecutive hemor
rhages from a bleeding duodenal
ulcer should be operated upon
without further ado.
The type of operation per
formed under the circumstances,
depends upon the degree of ob
struction, if any, involved. If no
obstruction exists, the nerve
cutting operation "vagotomy"
may be employed, either with or
without the creation of a new
outlet from the stomach. Severe
obstruction usually calls for re
moval of a considerable portion
of the stomach in order to pre
vent the formation of a new
ulcer from continuous acid secre
tion of the stomach with the
creation of a new outlet.
The results of this type of sur
gery are variable. In general,
they are very satisfactory. In
competent hands and with mod
eren methods of preparation and
post-operative management, the
hazard is slight. Surprisingly
slight handicap in eating or di
gestion follows. The patient
soon gains weight and recovers
vigor and zest for living.
However, these results are not
entirely uniiorm. The occasional
development of a "gastric crip
ple" following operation, has
moved some surgeons to adopt a
much more conservative attl
ture regarding surgical interfer
ence. They contend that if a per
son survived three hemorrhages
he can probably survive more.
Very often, ol course, a person
who has survived three hemor
rhages has had all he wants and
asks for surgical treatment.
Medical treatment of duodenal
ulcer, involving diet, rest, regu
lation of physical activities, med
ication and avoidance of emo
tional disturbance, has Droved
highly successful. If victims of
ulcer would louow me rules iaia
out for them, hemorrhages prob
ably would be far less frequent.
If a person Just simply will not
follow the rules if he wants to
eat his cake and have it sur
gery will appear more attractive
to him.
The person who once suffers a
hemorrhage,' and who upon re
covery embarks upon his old
habits of eating, drinking,
smoking, dissipation and general
disregard of the rules of health,
is asking for surgery. For sucn
a person, following his inira
hemorrhage, surgery is probably
the proper thing.
