Probably rheumatic fever to
day is a disease of late child
hood, adolescence, young adult
life in most cases, although the
first attack occurs before the
age of ten years in ten per cent
of the cases.
Osier ascribed rheumatic fe
ver to exposure, ' uccupaiiuim
which necessitate exposure to
cold and great changes of tem
perature predispose strongly . . .
Exposure to cold, a wetting, or
sudden change of temperature
are among the factors In deter
mining the onset of an attack,"
wrote the great medical teacher
of his day and here Is one rea
son why he was a Kreat teacher:
He adder, "but they were present
in only 12 per cent of our cases."
Evidently Dr. Oler was con
cerned about tlje other 88 per
cent of cases, wondering why
only one out of eight victims of
rheumatic fever could recall any
particular exposure to cold, wet,
sudden change. Virtually every
man, woman and child who is
not confined to bed is Inevitably
exposed to cold, wet, sudden
change of temperature, draft,
dampness, night air, fog, dew or
such such change of environ
ment daily If not hourly as long
as he is alive. I wonder what
ever Induced Dr. Osier to re
mark that exposure had nothing
to do with seven out of eight
rases of rheumatic fever. And I
wish the great teacher could
have had the grace (which comes
easier today) to concede that
even in the one case there was
no scientific proof that exposure
to cold, wet or sudden change
of temperature determined on
set of the attack; that that was
just an old medical theory con
ceived by doctors away back In
the dim past.
Dr. Osier defined rheumatic
fever as "an acute, non-contagious
fever, dependent upon an
unknown infective agent, and
characterized by multiple arth
ritis" (inflammation of many
joints). Although the joint in
flammation is painful enough
and the fever Is as serious as
fever from any other cause, it
Is not the illness Itself that gives
us so much concern, but rather
the complications which are li
able to occur, particularly in
flammation of the heart lining
(endocarditis). That will be the
subject of the next talk about
rheumatism.
