THE INFORMATION REQUESTED IN THIS SURVEY IS REQUIRED FOR COMPLIANCE WITH GENERAL REVENUE
SHARING AND ANTIRECESSION PROGRAMS.
Before filling out this form please read carefully each part and all related definitions, as well as any supplementary
reporting instructions that may be enclosed for your State.
Report all applicable revenue items listed, as well as all expenditures of yout government and its agencies. Include the
transactions of all funds - general fund, street and road funds, bond and interest funds, special assessment funds, utility
funds, and all other special and trust funds, i Exclude private trusts.)
Certain receipts and payments are net defined by the Bureau of the Census as revenues and expenditures, and should not
be so reported. These are: (a) refunds of revenues or expenditures; (b) moneys collected by your county acting only as
agent for the State or other governments f e.g., social security and income tax withholdings); (c) transfers of amounts
between funds, departments, or agencies of your government; and (d) receipts from sale of investments and payments for
purchase of investments.
The Bureau of the Census uses a separate questionnaire to collect information on pension funds administered by your
government. Therefore, exclude the activities of such funds from this report.
Please complete all parts of the form. Spaces for any items which do not apply to your government should be marked
with a dash or the word "None," please do not merely leave them blank. Do not delay reporting to await finally audited
figures if substantially accurate figures can be supplied on a preliminary basis.
