NOTICE OF BUDGET COMMITTEE MEETING
A public meeting of the Budget Committee of the Fox Cemetery
Maintenance District, Grant, State of Oregon, on the budget for
the fiscal year July 1, 2021 to June 30, 2022, will be held at 54888
Fox Valley Lane, Fox, OR. The meeting will take place on June 23,
2021 at 6:00 p.m.
The purpose of the meeting is to receive the budget message.
This is a public meeting where deliberation of the Budget Com-
mitee will take place. An additional, seperate meeting of the Bud-
get Committee will be held to take public comment. Any person
may appear at the meeting and discuss the proposed programs
with the Budget Committee. The meeting for public comment will
be on June 24, 2021 at 6:00 p.m., held at 54888 Fox Valley Lane,
Fox, OR. A copy of the budget document may be inspected or
obtained on or after June 23, 2021 at 54888 Fox Valley Lane, Fox,
between the hours of 6:00 p.m. and 8:00 p.m.

St. Charles Health System, Inc. (“St. Charles”) is providing this no-
tice pursuant to the Health Insurance Portability and Accountabil-
ity Act of 1996 (“HIPAA”) to inform St. Charles patients that one of
the health system’s data storage vendors, Elekta, Inc. (“Elekta”),
recently notified St. Charles of a security incident that may have
involved the personal information of some St. Charles’ patients.
This event did not occur at St. Charles, nor did it impact the secu-
rity of St. Charles’ computer network and systems.
St. Charles contracts with Elekta to provide cancer registry soft-
ware and data management. Though it was Elekta that suffered
the security incident, St. Charles is the entity that is subject to
the breach notification rules of HIPAA. As such, St. Charles is re-
quired to provide notice to patients whose personal health infor-
mation may have been accessed. This media notice explains what
happened at Elekta, the information that may have been affected,
measures Elekta is taking in response, and steps impacted pa-
tients can take to protect themselves.
What happened?
On May 14, 2021, Elekta notified St. Charles that certain of Elek-
ta’s network systems experienced a data security incident from
April 2, 2021 to April 6, 2021, and from April 19, 2021 to April 20,
2021. This event did not occur at St. Charles, nor did it impact the
security of St. Charles’ computer network and systems. Immedi-
ately upon detecting the incident, Elekta engaged expert forensic
investigators to launch an investigation to determine the nature
and scope of the incident. Elekta also contacted law enforcement
and has been cooperating with them since. When Elekta’s inves-
tigation revealed that the incident may have impacted personal
information of St. Charles patients, Elekta contacted St. Charles
and provided it with information regarding the nature and scope
of the incident, Elekta’s forensic investigation, and the potential-
ly impacted patient personal information. Elekta’s investigation to
date has revealed no evidence the personal information has been
disclosed publicly or used for any fraudulent purpose.
What information was involved?
From Elekta’s investigation, the following types of personal infor-
mation of certain of St. Charles’ Oregon patients may have been
impacted by the incident: full name, Social Security number, ad-
dress, date of birth, height, weight, medical diagnosis, medical
treatment details, and appointment confirmations. No financial
account, credit card, or debit card information was involved in this
incident. And, from Elekta’s forensic investigation to date, there is
no evidence to suggest that personal information has been dis-
closed publicly or used for any fraudulent purpose.
What Elekta and St. Charles are doing
Upon learning of this incident, Elekta immediately launched an
investigation and engaged the assistance of third-party forensic
experts. Elekta also took steps to prevent further access to in-
formation from its systems, including resetting passwords and
implementing multi-factor authentication. When St. Charles was
notified by Elekta of this incident, the health system commenced
an investigation of its own to determine what St. Charles data may
have been at risk, to understand the nature, scope, and cause of
the incident at Elekta, and to evaluate Elekta’s remediation mea-
sures. On June 14, 2021, St. Charles mailed written notification to
all impacted patients informing them of the Elekta security inci-
dent. As an added precaution, Elekta is also offering free access
to identity monitoring, fraud consultation, and identity theft resto-
ration services to any impacted St. Charles patients.
What impacted patients can do now
Although St. Charles has no evidence that any personal informa-
tion has been misused, the health system urges impacted patients
to take advantage of the free credit monitoring and identity theft
protection services being offered by Experian in the notification
letters sent by U.S. mail on June 14, 2021, and encourages individ-
uals to remain vigilant by reviewing account statements and credit
reports closely. The notification letters contain additional informa-
tion at the end regarding steps that impacted patients can take
to further protect their personal information, including(1) reporting
suspicious activity or suspected identity theft, (2) credit reports,
and (3) credit/security freezes.

Grant County has acquired title to the properties described on the attached Exhibit “A” through tax foreclosure
proceedings or otherwise; title is now vested in Grant County and it is in the best interest of Grant County to sell
