List in no more than two sentences FIVE WAYS your unit makes people feel better, get better, or access health
care services:
1.
2.
3.
4.
5.
We are the “Go-Between” between patient and their doctor. If we see a need we contact the doctors so the patient doesn’t
need to make an appointment or even leave their home. We report the s/s that we are seeing and get orders for treatment.
We review patients home for safety issues and make recommendations to patients and family.
We bring many skilled services to the patient’s home. We provide Nursing, Aide, Physical Therapy, Occupational Therapy,
Speech, and Dietary
We can provide IV therapy in the home. Long term IV use managed in the home makes this easier for the patient.
We have an on-staff wound certified nurse and people with complicated wounds can stay at home instead of hospital.
Explain in no more than three sentences why Grant County residents should pick YOUR unit to obtain the service.
We have well-seasoned Grant County nurses who love to serve people in their homes and not afraid to drive in the Grant County
weather. We provide standard of care in home health and help people get back to their pre-illness, pre-injury state.
For your unit, describe who typically pays (is it insurance? self pay? a split? other?) because people often
don’t understand this. If they had a better sense, for instance, that insurance covers your service, they more
likely will use it.
We do receive payment from insurance.
