COMMUNITY SCREEN WEEK
November 14-18, 2016
FOR CHILDREN 2 MONTHS – 5 YEARS OLD
Will your child be left behind? Does your child struggle with
any areas of development? Will your child be ready for
kindergarten? If children enter kindergarten behind their peers and do
not “catch up” by third grade, it is unlikely that they will have a successful
educational experience. Screening young children is an effective way to
catch problems and provide parents and children with needed skills when
it does the most good - during the crucial early years when the child’s
brain and body are developing so rapidly.
Screeners will use the Ages & Stages Questionnaire
(ASQ) to look at cognitive, fine motor social/
emotional, gross motor, and communication skills.

Since 2013 with all the changes and requirements in healthcare, Blue Mountain Hospital
District (BMH) has been offering the community assistance with this confusing process.
Elizabeth Pace and Pam Wilson are certified through the State of Oregon as Application
Assistors. As Application Assistors they are trained to help with Qualified Health Plans through
the federal website Healthcare.gov.
This service is offered free of charge to our community and it
takes the confusion and hassle out of the lengthy process.
During open enrollment people are able to go online, sign up and apply for health insurance.
The benefit of the federal website is that it is the only way you can apply for premium tax credits
and cost sharing benefits. Elizabeth and Pam are also trained and certified to help patients with
low income health insurance through the Oregon Health Plan.
If a patient comes in with no insurance and immediate medical needs,
Assistors are able to determine eligibility and if a patient qualifies get them
immediate limited coverage through the Presumptive Application process.
Applications are available at BMH and Assistors offer assistance in filling them out. They can
copy any needed information required by the State of Oregon and fax your application in.
Faxing your application eliminates them being lost in the mail and they arrive sooner.
Assistors are able to make phone calls on your behalf regarding
application status, and make retro requests if you have previous
medical bills.
Starting January 2017, there will be a new website for the Oregon Health Plan that will be
open to the public.
