NOV. 9
• Vehicle theft was report-
ed on the 900 block of North
Douglas Street.
• A mental hold was
placed on an individual in
Coiner Park with the help
of a crisis response mental
health worker. The individ-
ual was transported to the
University District Peach
Health Hospital.
• An individual reported
having his car window bro-
ken overnight on the 1100
block of Edgewater Lane at
his home and a backpack
stolen from the vehicle
which contained a laptop
and an iPad.
NOV. 10
• A general contractor
reported theft from a con-
struction site located on
Gateway Blvd. A respond-
ing officer was advised how
an unknown suspect had
cut a section of chain link
fence and unlawfully en-
tered the otherwise secured
and fenced construction
site. The complainant said a
compactor was stolen some-
time between the dates of
Nov. 4 and Nov. 10. Costs
to replace were estimated at
approximately $1,700. The
complainant advised that
it would have likely taken
multiple people to lift and
remove from the property.
• Fraud was reported at a
Gateway Blvd store when it
was discovered a $50 bill ac-
cepted at an unknown time
was counterfeit.
• Fraud was reported at
Bi-Mart in regards to a $100
counterfeit bill which had
been attempted to be ten-
dered during a purchase. A
suspect was positively iden-
tified and told responding
officers that he did not know
the bill was fake, though it
displayed “neon pink-col-
ored Chinese markings on
the back and front.” The bill
was seized as evidence but
no charges were pursued.
NOV. 11
• Officers responded to a
call of a burglary at The Sen-
tinel office on Main Street.
The case is still under inves-
tigation.
• CGPD dispatch received
multiple 911 calls from a fe-
male who stated while driv-
ing she was suicidal and was
contemplating overdosing.
Officers spent some time
trying to locate the person
as she continued to drive
in the area and ultimately
located the vehicle, contact-
ed the driver and gave her
a courtesy ride home. The
case was eventually cleared
after determining she did
not need to be put on a hold.
• CGPD dispatch advised
of a single vehicle crash into
the river on Mosby Creek
Road, with one subject out-
side of the vehicle acting
dazed. A responding officer
attempted to determine if
there were any unaccount-
ed for people in the vehicle
in the river. While en route,
CGPD dispatch received an
additional 911 call advising
that the driver of the crashed
vehicle stole the vehicle of
the original caller, possibly
at gun point, and left the lo-
cation east on Mosby Creek.
Upon arrival, there was
a female standing on the
roadway, in a bathrobe, flag-
ging down an officer with a
flashlight. The entire west
bound lane was blocked
with what was determined
to be cable lines and a cable
pole that was sheered and in
the roadway, still attached to
another pole on the south
side of the road, making
any passage east bound also
very limited. The female ad-
vised that her husband was
the original caller and when
the suspect stole his vehicle,
the husband got their oth-
er vehicle and began trying
to find the suspect, but did
not have a cell phone with
him. The female and anoth-
er neighbor pointed an offi-
cer to where the originally

crashed vehicle went off the
north side of the road, strik-
ing a tree and cartwheeled
into the river, halfway sub-
merged. It was determined
no one was inside the vehi-
cle, but both air bags were
deployed, the trunk was
ajar and the passenger front
window was smashed out,
“likely for the self-extrica-
tion.” Lane County author-
ities arrived and took over
the scene, while the other
units responded further east
to attempt to locate the sus-
pect and stolen vehicle. Due
to the location (approxi-
mately mile post 4) being a
sharp corner and both lanes
blocked, an officer stayed
to assist with traffic. EPUD
arrived and was able to
pull the lines into the west
bound lane.

• A male suspect was ar-
rested on the 800 block of
North Lane Street for As-
sault in the Second Degree,
Unlawful Use of a Weapon
and Menacing when he in-
tentionally struck a female
with an aluminum baseball
bat on the right forearm,
causing physical injury.

David Michael Perkins
1970-2021
David Michael Perkins,
51, of Cottage Grove, Oregon
passed away November 7,
2021. David was born to par-
ents Fred and Sandra (Olson)
Perkins October 21, 1970, in
Cottage Grove. He graduated
from Elmira High School in
1988. David attended Oregon
Poly Technical Institute and
Lane Community College re-
ceiving his associates degree
at both schools. David was a
huge Oregon Ducks football
fan and enjoyed playing racket
ball, pickle ball, and coaching
youth sports. He loved the out-
doors spending time camping,
fi shing, and seeing Oregon’s
beautiful lakes and rivers in
the summertime. His greatest
joy was spending time with his
niece and nephew Dylan and
Morgan. David was a fi erce
loyal brother and friend. He
was a member of Emerald Fit-
ness Club, and Cottage Grove
Faith Center. David was well
loved and respected in the
community for his kind, gen-
tle and authentic nature. He is
survived by his mother Sandra,
and stepdad Dan Davis; broth-
er Tom Perkins of Cottage
Grove; nephew Dylan; niece
Morgan and numerous ex-
tended families. He is preced-
ed in death by his father Fred
Perkins. A service will be held
November 20, 2021, at 1:00PM
at the Cottage Grove Faith
Center. He will truly be missed
by his family and friends. Ar-

Rene Cook-Taylor
1958-2021
Rene’ answered heavens call
and is now walking alongside
the angels. Her memory will
continue to beat in our hearts
as we mourn the loss of her
battle with cancer. Rene’ Mi-
chelle Cook went home to the
Lord on the evening of Octo-
ber 30th, 2021. She was born
October 14, 1958, to Richard
and Diane (Vogt) Lasslett in
Garden City, Michigan.
She was an active member
at Riverside Church of God
and Calvary Chapel in Cot-
tage Grove. She loved spending
time with her family, playing
with her grandchildren, cook-
ing and baking, aerobics class-
es, sewing, craft ing, renovat-
ing, playing bingo, gardening,
boating and going to Oregon
Duck football games.
Rene` is survived by James
Cook, her children; Stephen
Markman, Taylor cook and
Amanda Borrelli; her sisters
Jeanine Toney and Dawn Gar-
cia; her brother Ricky Lasslett;
3 grandchildren, Brynlee, Ry-
der, Mila and Elijah is on the
way.
We know that Rene’ touched
the lives of many, she was al-
ways laughing, smiling, and
joking around. She was an
amazing Wife, mother, and
Grandmother. She loved her
family and those around her
fi ercely and unconditional-
ly. A life so beautifully lived
deserves to be beautifully re-
membered. Please join us on

In 1994 Tom divorced Barba-
ra. He married Marsha Watson
but this marriage was short-
lived. In 2000 Tom married
Toni Marie Spencer (nee Car-
rano). He became stepfather to
Josiah, Hannah, and Rebekah,
and later a grandfather to Sele-
na, Isaac, Leo, Zayah, and Ares.
Tom loved reading and read
biographies of every US presi-
dent. He enthusiastically played
guitar adequately, walked every
day, enjoyed bridge and other
games, traveled to Europe six
times, and obsessively collect-
ed recorded music. Tom loved
his life. His life was enhanced
by his relationships with many
mentors, friends, colleagues,
and clients. He was gratifi ed
to be able to help others in his
work and to be a good support
to Toni and the kids. Funeral
Service was held on Monday,
November 15th, 2021 at Smith-
Lund-Mills Funeral Chapel in
Cottage Grove, Oregon with a
private interment at Fir Grove
Cemetery.

WWII, Kenneth was stationed
on the destroyer USS Frank E
Evans to the end of the war.
On June 22, 1947 he married
his wife Elaine (Summerford).
Aft er the war he went to work
for the City of Torrance, CA
4 grandchildren: Joy (Chris),
Kerri (Nathan), Ryan (Jenny)
and Kimberly (Jeremy), and 3
great-grandchildren: Zachary,
Brooklyn and Presley. A sis-
ter-in-law Donnette, a niece
Stephanie (Troy) and their
son Chase. A memorial ser-
vice will be held at the VFW
Post 3473 in Cottage Grove on
December 11th at 1:00 p.m.

supervise including trash, city
yard, and signs and signals. He
retired in 1977 and moved to
Cottage Grove. He loved living
in Cottage Grove, and working
Michael Paul Matchulat
1984-2021
Michael pursued a number
of jobs before fi nding a home
in law enforcement. He served
with the Lane County Sher-
iff ’s Offi ce until his cancer di-
agnosis forced him into early
retirement.
1954-2021
as an economist, and went to
work on a psychiatric ward of
a local hospital. Here he found
his calling. He earned a MA in
rehabilitation counseling, then
moved to Eastern Kentucky for
his fi rst job as a counselor. At
this time he married his fi rst
wife, Barbara Cassin, and their
marriage lasted 16 years.
Tom and Barb moved to
Bend, Oregon in 1981 and he
worked for Vocational Reha-
bilitation Division, and then
moved to Eugene in 1984.
Th ere he worked for Laurel Hill
Center, helping adults with se-
vere psychiatric disabilities live
and socialize in the community.
While at LHC he built Laurel
Grove Apartments.
In 1992 he went to work for
South Lane Mental Health in
Cottage Grove. At the time
SLMH was a small struggling
non-profi t. Over the next 25
years Tom and his staff built a
mission-based
organization,
each year helping more than
1,000 children and adults ad-
dress their mental health con-
cerns. Tom loved this work
and was proud his organization
was able to serve his newfound
community.
Although he was glad he
didn’t work as an economist
he had a lifelong passion for
for 20 years. Ken is preceded

Th omas Alan Wheeler
Th omas Alan Wheeler was
born on December 5, 1954 in
Louisville, Kentucky to Do-
ris (nee Stockhoff ) and Wil-
liam Charles Wheeler. He had
one brother, Bill (and his wife
Jackie). Tom was raised in a
comfortable setting with great
grandparents, Fred and Elsie
Stockhoff and Mayme Wheeler
nearby. He attended incredible
public schools, was active in
church and Boy Scouts, played
various sports, and vacationed
throughout the US with his
family.
Tom graduated with a BA
in Economics from the Uni-
versity of Kentucky but spent
the majority of his time with
a group of friends in all stages
of production of rock and jazz
concerts for the university. By
the time he graduated Tom
knew he wouldn’t be happy

Michael Paul Matchulat was
called to heaven from his Lo-
rane home early on the morn-
ing of November 11th, fol-
lowing a 6 1/2 year battle with
cancer. He was 37 years old.
Michael was born in Lo-
rane, Oregon where he grew
up on the family farm with
his parents, Paul, and Trish
Matchulat, and his four older
sisters. Family and commu-
nity shaped who Michael be-
came.
Michael attended Crow
High School as a gift ed musi-
cian and a multi-sport athlete.
He graduated from CHS with
honors in 2002, before going
on to earn a B.S. from the Uni-
versity of Oregon in 2006.
In 2015, Michael married
the love of his life, Amber
Dawn Doughty. Together,
they built a life and home sur-
rounded by natural beauty.
Th ey allowed each other to
reach their full potential and
encouraged others to do the
same.
He was a peace offi cer, a
farmer, a hunter, and a welder.
He was a horseman, a build-
er, and a mechanic. He had
a deep appreciation for fi ne
wine, cooking, sunsets, mush-
room foraging, scuba diving,
fi shing and most of all, people.
He was an artist, an author, a
musician, and a marksman.
Michael was known for his
eclectic style. He thrived on
eccentricity and valued hon-
esty. He kept you guessing.
One never knew if he would
show up wearing a three-piece
suit or daisy duke cutoff s. On
one occasion, he drove up in
his Caddy with a new calf in
the backseat.
Michael was a member of
the Lorane Christian Church
and his faith remained strong
to the end. He loved life, his
family, his friends, and his
community. He was a beloved
son, husband, brother, uncle,
and true friend to many.
Michael was preceded in
death by his father, Paul. He
is remembered by his moth-
er Trish, his wife Amber and
his sisters Laura, Michelle,
Melinda and Amber and his
brothers (in law and in spir-
it) Ryan, Jeff , James, Justin,
and Duncan. His nephew and
seven nieces will be telling his
stories to their children.
