Oregon Department of Human Services notifies public of data breach
SALEM—Th e
Oregon
Department of Human Ser-
vices uncovered a phishing
incident that aff ected email
records at the department.
Unfortunately, Protected
Health Information under
the Health Insurance Porta-
bility and Accountability Act
(HIPAA) was compromised
and potentially exposed.
Th e agency has hired an
outside entity, IDExperts, to
perform a forensic review to
clarify the number and iden-
tities of Oregonians whose
information was exposed,
and the specifi c kinds of in-
formation involved.
Th e Department of Hu-
man Services takes privacy
and the confi dentiality of
client information seriously
and has strong information
technology security process-
es in place, which enabled
the department to detect and
contain the incident. Th e
department cannot confi rm
that any clients’ personal in-
formation was acquired from
its email system or used in-
appropriately.
However, it is notifying
the public because infor-
mation was accessible to an
unauthorized person or per-
sons.
Although DHS has not
confi rmed that clients’ per-
sonal information was ac-
quired during the incident,
DHS considers the inci-
dent a breach under Ore-
gon’s Identity Th eft Protec-
tion Act (ORS 646A.600 to
646A.628).
Th erefore, this notifi ca-
tion is provided in part as a
substitute notice of a breach
under Oregon’s Identity
Th eft Protection Act, because
the class of aff ected consum-
ers exceeds 350,000.
Th e facts are summarized
below, along with protective
measures the department has
taken since discovering the
incident and general guid-
ance on protecting personal
information.
What happened?
On Jan. 28, DHS and En-
terprise Security Offi ce Cy-
ber Security team confi rmed
that a breach of regulated
information had occurred.
Nine individual employees
opened a phishing email and
clicked on a link that com-
promised their email mail-
boxes and allowed access to
these employees’ email infor-
mation.
Current information in-
dicates on Jan. 8, a spear
phishing email was sent to
DHS employees. Th rough
our process of discovery, we
learned that there were near-
ly 2 million emails in those
email mailboxes.
Th e unauthorized ac-
cess to the aff ected email
mailboxes was successfully
stopped. DHS is in the pro-
cess of thoroughly reviewing
the incident and the infor-
mation involved. Th is inves-
tigation includes clarifying
the number of impacted rec-
ords that might contain per-
sonal information of clients
receiving services from DHS.
What information was
involved?
Clients’ Protected Health
Information
under
the
Health Insurance Portability
and Accountability Act (HI-
PAA) was accessible to an
unauthorized person. Client
information may include fi rst
and last names, addresses,
dates of birth, Social Security
numbers, case number and
other information used to
administer DHS programs.
Human Services doing?
Th e security and con-
fi dentiality of personal in-
formation is critical to the
Department of Human Ser-
vices. While there is no in-
dication that any personal
information was copied from
its email system or used in-
appropriately, the depart-
ment will be off ering identity
theft recovery services for
impacted individuals.
DHS is in the process of
determining whose infor-
mation was aff ected by this
breach.
Once confi rmed, IDEx-
perts will send individual
notices to identifi ed individ-
uals, including notices to cli-
ents whose HIPAA-protect-
ed information was involved,
with instructions on how
to register for the service,
which includes free credit
monitoring.

day (March 22, 2019) at 800-

ents with more information.

Department of Revenue urges Oregonians to be aware of new tax scams
It’s never safe to let down
your guard, warns the Ore-
gon Department of Revenue
(ODR). Scam tactics are al-
ways evolving and becoming
more eff ective. Scammers try
many diff erent methods to
trick people into giving them
personal information or mon-
ey.
Th e best way for taxpayers
to make payments directly to
the ODR is through Revenue
Online. It’s secure and includes
all the information necessary
to ensure the payment is prop-
erly applied to the correct ac-
count.
Go directly to oregon.gov/
dor to fi nd Revenue Online.
Payment providers may pro-
vide links that appear to take
you to the government site, but
just end up taking you to an-
other area of the provider’s site.
Scams mainly come in the
form of a phone call, email, or
standard mail. Here are some
tips to help you identify scam
attempts.
• Scammers make unsolicit-
ed calls. Th ieves call taxpayers
claiming to be representatives
of the Oregon Department of
Revenue or other tax offi cials.
Th ey demand that the victim
pay a bogus tax bill and may
use threats or a sense of ur-
gency to con the victim into
sending cash, usually through
a prepaid debit card or wire
transfer. Th e Department of
Revenue never uses methods
like these when making calls.
Hang up on suspicious
phone calls. No matter how
urgent a message makes a sit-
uation sound, you can always
hang up, call the Department
of Revenue at their published
phone numbers [503-378-4988
or 800-356-4222 (toll-free)],
and know that you’re dealing
with an actual government
employee.
• Scammers send letters.
Letters oft en contain legitimate
logos, addresses, and phone
numbers to fool you. Some-
times, these letters expose
themselves as scams through
blurry logos, misspellings, and
poor grammar.
Letters are usually in the
form of a fake tax bill or claim-
ing an error with your account.
Letters from the Department
of Revenue will have informa-
tion that is verifi able through
our website and every letter is
printed with an identifi cation
number in the upper right cor-
ner.
• Scammers set up fake
websites. Some scams that
start as unsolicited calls or let-
ters may also try to send you to

of Cottage Grove, Oregon
passed away March 18, 2019.
He was born in Knappton, WA
to Omar and Bethel (Skinner)
Harwood. Neil joined the Unit-
ed States Army and worked as a
surgical technician. He married
Rachel Bessonette July 28, 1947
in Deadwood, OR. Th ey were
married 69 years. Neil worked
logging related jobs, and for
many years as a road grader
operator in Mapleton, OR. He
enjoyed family and friends. Neil
loved camping and exploring
many places in Oregon. He also,
enjoyed having a full wood shed
for cozy fi res in his fi replace.
He is survived by son David
(Janette) Harwood of Walter-
ville, OR; son Jeff rey Harwood
of Cottage Grove, OR; brother
Oscar Harwood of Amity, OR;
brother Rex Harwood of Weiser,

Woolcott died peacefully
the morning of March 19 at the
age of 93. She was born June 25,
1925 in Hood River to Fred and
Zana (Lee) Carnes and was pre-
ceded in death by her husband
of over 55 years, Russ Woolcott.
June was raised in Hood Riv-
er, graduated from University
of Oregon, and taught PE/En-
glish at Grants Pass and Cottage
Grove High Schools. She taught
Sunday School at the United
Methodist Church (CG); was
an active member of the Amer-
ican Legion Women’s Auxiliary,
Julianna Garden Club, the PTA,
and First United Methodist
Church (Eugene); was both
a Cub and Girl Scout leader;
played bridge and pinochle; and
was on a bowling team spon-
sored by Randall & Woolcott.
She loved music, gardening,
sports (especially basketball),
and Mt. Hood.
Survivors include children
Chuck (Dawn) Woolcott, Gail
(Parke) Ball, and Sally (Bob)
Schroeder; grandchildren: Sara
(Kyle) Piper, Kelsy (Allen) Foul-
stone, Nicole (David) Cline,
Grant (fi ancé Samantha) Schro-
eder, Victoria (Devin) Geiger,
Emma Woolcott, and Evan
Woolcott; and great-grandchil-

Grove, Oregon passed away
March 18, 2019. He was born
October 3, 1933, in Denver,
CO to parents Henry and Adda
(Halter) Westerkamp. Norman
graduated in 1951 from North
Denver High School, and Rocky
Mountain Technical Institute
from 1957-1961. He served in
the United States Navy on the
USS Saint Paul during the Ko-
rean War. He married Cornelia
T. Lowrance, November 16,
1954, in Westminster, Colorado.
Norman worked as a Custodi-
an in Denver public schools.
He also worked as a pin setter
at a bowling alley, at stockman
Dixon Paper, and Jeff erson
County Schools as an Electri-
cian Apprentice. He owned and
operated NorWest Backhoe and
Septic Services until his retire-
ment in 1995. Normans hobbies
included being on the pit crew
for Googie Gallow. Norman is
survived by his wife Cornelia,
daughter Carol (Gerald) Water-
house; daughter Karen (Ronald)
Perkins; son Bobby (Melanie)
Westerkamp; son Clent (Cheri)
Westerkamp; son Mark (Barba-
ra) Westerkamp; 10 grandchil-
dren; 2 great-grandchildren; 1
great-great grandchild. He is
preceded in death by daugh-
ter-in law Lisa Williams (West-
erkamp). A graveside service
