CTGR 2018-19 K5 Afterschool Program
Applications will be available on Monday July 5 at Youth Education
Applications will not be accepted before the above posted open enrollment date or during
non-office hours. Submitted applications must be completely filled out and include a copy of the
child’s Tribal ID or C.I.B. If the child is a CTGR Direct-descendant, a copy of their parent’s Tribal ID
or C.I.B. will need to be included.

*Please note: If a waiting list is needed at the conclusion of the sign-up week on August 3,
Tribal member youth signing up after the program enrollment is at capacity, will be placed at the
top of the wait list .
Location is at the CTGR Youth Education buildings
For Who: Enrolled CTGR Tribal youth, CTGR Tribal descendant youth and other
enrolled Native American students who are entering Kindergarten through 5th
grade in the Willamina and Sheridan School Districts this fall (please note that we
are limited to 10 youth from Sheridan due to transportation limitations)
Transportation is provided on a CTGR bus from
Willamina Elementary School. Due to van seating
limitations, we can only serve 10 students from
the Sheridan Elementary Schools (FCS and SJS).
Priority Tier List:
#1- CTGR Tribal members
#2- CTGR direct-descendants
#3- Members of other Federally-Recognized Native American Tribes
#4- NonNative sibling member of a Native family
Provide healthy afternoon snacks each day and a
variety of educational, recreational, and cultural
enrichment activities.
Limited spaces available
K5 Afterschool roster spots are filled on a first come, first serve basis using a
“Four Tier System,” with 60 roster openings, (K/2: 30 spots & 3/5: 30 spots)
Time for K5 Afterschool is 3:45–5:15 p.m., plus a
few full days throughout the year.
