This matter came before the Court on February 8, 2016 by way
of Petitioner’s request to amend the Order Granting Request to
Change Name due to a clerical error in the spelling of Ms. Duran’s
middle name in the original Order. Upon review of the case fi le
and February 8, 2016 request, the Court fi nds good cause to amend
the February 4, 2016 Order Granting Request to Change Name as
Follows:
NOW THEREFORE, IT IS ORDERED:
1. The name of the Petitioner is changed from Stephanie Mae
Duran to Stephanie Mae Jackson;
2. The Clerk of the Tribal Court shall submit a certifi ed copy of
this Amended Order to the Grand Ronde Tribal Enrollment offi ce;
3. The Clerk of the Court shall provide a certifi ed copy of the
Amended Order to the Oregon State Department of Vital Statistics.
