If client has not worked for a period of time, client will need to submit a printout from their State Employment Offi ce or printout from Oregon Self-Suffi ciency Offi ce as verifi cation of income. Rental/deposit or utility assistance may be utilized only one time in a 24-month rolling calendar period and is not to exceed the maximum contribution of $1,000 per household within any 24-month rolling calendar period. Medical Gas Vouchers Social Services will not provide gas vouchers for medical ap- pointments, counseling or drug and alcohol treatment. The only exception is for scheduled treatment plans (i.e.: radiation, chemo- therapy, surgery, etc.) that have caused hardship on the family. Medical gas vouchers can be distributed to meet the need of the circumstances. A copy of all scheduled treatment plans must be received before client is eligible to receive this service. Of the 1,108 who registered, 630, or 56.8 percent, voted. Tribal voters endorsed the pri- mary election idea 408-222, but it fell 2 percentage points shy of the two-thirds approval required to amend the Tribal Constitution. Offi cial BIA ballots will be mailed out to registered Tribal voters on Thursday, May 22. Ballots must be returned to the BIA’s Siletz Agency offi ce no later than noon Friday, June 6, to count. PROJECT DESCRIPTION The Grand Ronde Food Bank project located in Grand Ronde, Ore., includes the construction of a 3,300- square-foot food distribution center located at 9675 Grand Ronde Road in Grand Ronde, Ore. TRIBAL EMPLOYMENT RIGHTS ORDINANCE (TERO) AP- PLICABILITY The Tribe has adopted a TERO Ordinance which ap- plies to this project. The selected contractor will be a Covered Employer and subject to TERO requirements. (See TERO Notice included in Section 00 75 00). As part of these requirements, Contractor will be subject to a Compli- ance Plan setting forth preferences in employment and contracting, as well as an apprenticeship program requirement. The contract will be subject to a 2.5% TERO fee. The selected contractor must attend the next contractor certifi cation training offered by the TERO Offi ce which is expected to occur in July. INDIAN PREFERENCE Any proposals submitted by an Indian Owned Business (IOB) within fi ve percent (5%) of the highest scoring proposal will be awarded preference in accordance with the TERO Ordinance.
