To find out if you are eligible, or to get enrolled in Medicare Tart H, please do the following: call toll-free 1-800-MEDICARK (1-800-633-4227) or go online at www.medicare.gov. You will need to request an application for enrollment in Medicare I 'art B and return it to your local Social Security Office. Once you are enrolled in Medicare I'art B through Social Secu rity, you will niH.fl to call our office to request a Medicare I'art B lieimbursement packet to apply. As a Grand Kondc Tribal member, the Tribe will reimburse your Medicare I'art B premium each month at the standard rate. The Confederated Tribes of Grand Rond Youth Education Program is inviting artists to apply to participate in a Public Mural project to enhance the program's new Youth Education Expansion and promote a healthy, drug-free youth facility. The Mural Project is intended to enhance the aesthetics of the Youth Education Facility, to reflect the pride in community, history and the cul ture of the Grand Ronde people and to promote positive living throughout the community. When designing your artwork, consider its contribution to the community as a whole. Successful proposals will promote goodwill and become a source of community pride by encouraging youth to lead healthy lifestyles. Mural requirements Mural will require the ability to be mounted to the existing exterior wall as opposed to being painted directly on to the wall. Project Dimensions The proposed area of area for the mural installation is an exterior covered area on the South Wall of the Youth Education Facility, roughly 9ft high X 30ft long (measurements to not reflect the size requirements of the mural, but must be confined within this space) The selected artist will be invited to one or more informational ses sions for an opportunity to meet with staffstudents, explore resources, collaborate, in order to make the project a successful one. Artists may submit more than one proposal and will be juried on the following criteria: Concept description and creative vision (50 points) Feasibility of project completion (15 points) Quality of prior work (25 points) Native Preference (5 points) Appropriateness of work to location (5 points) Timeline July 25, 2012: Submissions due July 30, 2012: Artists notified Aug. 3, 2012: Informational session with selected artist Sept. 25, 2012: Project completion Selection Committee The Program staff and students will serve as the Selection Committee.
