Dear Smoke Signals:
The Simmons family would like to send a letter of thanks to the Muck
leshoot and Grand Ronde communities for our recent gathering for the
Simmons Family Memorial and headstone blessings for Jimi Dexter Sim
mons, John Henry Simmons, Michael Simmons, Rosemary Dilling-Sim-mons,
Marcus Simmons and LaSage Simmons in Grand Ronde, as well
as the blessing for our mother-grandmother Marie Daniels, who rests
there as well.
We are thankful and grateful for the cooks, the Muckleshoot Canoe
Family, the Grand Ronde Canoe Family and Shakers whom traveled all
the day down to help with this wonderful healing ceremony.
Also, we would like to thank Grand Ronde for the use of their longhouse,
Achaf-hammi, their spiritual leaders, their Elders, our cousins for shar
ing their stories and all their help that made this occasion that will be
remembered for years to come.
We would like to thank the Muckleshoot Tribal Council, the Grand Ronde
Tribal Council and the Muckleshoot CeremonialWildlife Committee for
providing the elk and salmon that nourished our bodies.
There were a lot of good stories brought forward and shared with all.
It is a testimonial to the wonderful changes going on in the Muckleshoot
and Grand Ronde communities.

Tribal Elders are regularly seeking donations for fundraising garage
sales.
To donate an item or items, contact Tribal Elders Linda Brandon at

Changes to your
SkooEiunn Health Plan
For Tribal members residing in the counties of Washington, Mult
nomah, Tillamook, Yamhill, Polk and Marion, you will soon be receiving
a new Skookum card with changes to your health plan ID number. This
does NOT affect Tribal employees with the Shasta employee plan. We
are making a few changes to your health plan group identifier, which
means that you will need a new ID card.
The first three letters of your current ID number will be changing from
GRT to GRC.
o Your current ID number looks like: GRT123456.
o Your new ID number will look like: GRC123456.
Your ID number affects how your claims are paid, so to ensure your
claims are paid correctly, please THROW OUT your old card and KEEP
the new card starting with the GRC ID number.
o When you go to a doctor, dentist, pharmacy or eye doctor, make
sure you give the provider your new ID card so the bill will be
processed for payment correctly.
The final change relates to your Explanation of Benefits (EOB). Going
forward you will only receive an EOB if you owe the provider a portion
of the claim costs or if the claim has been denied.

Administrative Services' Customer Service at 1-800-880-5877 or Con
tract Health Services at 1-800-775-0095.
Again, these changes only affect Tribal members residing in. Washing
ton, Multnomah, Tillamook, Yamhill, Polk and Marion counties! This
does not affect Tribal employees.
Transportation
