Wellness Clinic At The Portland Satellite Office Wednesday, November 15, 2006 11 a.m. - 2 p.m. Grand Ronde Wellness Staff will be here to check blood sugar and blood pressure. The Tribal Foster Care Program will be in the Portland of fice every second Thursday of each month. We will be mak ing ourselves available to service our Portland area families, as well as recruitment. Kelly Ave. Portland, OR 97201 Phone: 503-235-4230 Fax: 503-239-8047 2006 Tribal Dividend Checks We would like to inform all Tribal members of the process that has been initiated at the casino for you to cash your checks with the greatest amount of speed and efficiency including the identification that will be required. There has been a change made to the location from the last article. On Thursday, December 14, we will have six windows set up in Kalapuya Banquet Rooms A & B for your convenience. Please use the parking lot behind the Lodge which will provide the easiest access through the white tent to the Tribal check cashing area. In order to simplify the handling of checks for all tribal members, four of the windows will be divided up by the first letter of your last name. The fifth window will be designated for Tribal Elders. The sixth window will be for checks that are a different amount because taxes or payments have been deducted. These windows will be ready to assist you in the cashing of your checks quickly and easily. Please bring your valid Tribal ID card or other valid proof of iden tification such as a Drivers License. You will also be asked for your current physical address as this is a requirement by the IRS.
