In the Matter of: VANPELT CASE NO. C-04-04-001
Meia Launie Ann Lloyd VanPelt DECREE CHANGING NAME
Petitioner
Based on the allegations set forth in the petition for change of name filed
by the petitioner in the above entitled case and its supporting affidavit,
THE COURT FINDS:
1. The petitioner is over 18 years of age;
2. The petitioner is an enrolled member of the Confederated Tribes of
the Grand Ronde Community of Oregon;
3. The petitioner has not petitioned and been granted a change of name
by the Tribal Court within the past twelve (12) months;
4. All persons entitled to notice have been properly served with a copy of
the Notice for Name Change, and;
5. No objection to the petition has been filed by any creditor or former
spouse of the petitioner.
Based on the foregoing findings THE COURT DECREES:
1. That the name of the petitioner be changed from Meia Launie Ann
Lloyd VanPelt to Melanie Meia Ann Sage Bear :
2. That the Clerk of the Tribal Court shall cause a copy of this order to
be published in the Tribal Newsletter;
3. That the Clerk of the Tribal Court shall submit a certified copy of
this order to the TribalEnrollment office;
4. That the petitioner shall surrender all old Tribal Identification cards
bearing petitioner's former name to the Tribal Enrollment office prior to
the Enrollment office issuing petitioner new Tribal Identification cards
bearing the petitioner's new name;
5. That within sixty (60) days from the date of this order the Tribal
Enrollment staff and if necessary the Tribal Enrollment Committee shall
cause new Tribal Identification cards to be issued to petitioner showing
the new name of the Tribal member and shall change the official member
ship roll and all official enrollment records of the Tribe to reflect the
petitioner's new name as stated in this Decree;
6. That the Oregon Department of Vital Statistics is authorized to
issue a new birth certificate to petitioner in such form as to indicate that
her name is Melanie Meia Ann Sage Bear.

ARTISTS WITHOUT BARRIERS
Who, What? A Non-Profit Organization has been set up to teach children art.
We charge $15 a month for art lessons, all supplies included. We take children
ages 5 to 16 years old. Lessons include drawing, pencil, pen & ink, watercolor,
acrylic, goush (a water base) and some calligraphy.
Where? Grand Ronde Church and Willamina Church
When? Mondays at Grand Ronde 1-3 p.m. Mondays at Willamina 4-6 p.m.
Teacher is Richard Fenwick. Our goal is to reach as many children as pos
sible. If you are interested please call Elverta Hayden Administrator at 503-390-4615.
The Tribal Council plans to allocate $500.00 to each member through the Member
Benefit Program for the year 2004. AH members 21 years of age on or be
fore the date of distribution are eligible for withdrawing. For all mer
bers under the age of 21. the distribution will be deposited Into the
trust accounts. For those eligible, withdrawal requests received in our offices
by June 14, 2004, will generate checks to be mailed on June 28, 2004. With
drawal requests postmarked June 14, 2004 or later WILL NOT meet the deadline,
they must be In our offices by June 14, 2004.
UNDER NO CIRCUMSTANCES MAY
CHECKS BE PICKED UP AT THE OFFICES.
Checks will be mailed to the address on the withdrawal form. Once the re
quest is received, the address for the check will not be able to be changed.
The address on the withdrawal form will be the official address for you, and all
other mail will also be mailed to that address. If you need the check to go to an
address other than the one on the form, please go to the post office in your area
and have your mail forwarded.
You will be receiving a withdrawal form in the mail regarding this distribution.
Also, there is a withdrawal form in Smoke Signals that you may use. This will be the
only payment date for the month of June. If you have a balance in your account,
you may request that on this withdrawal form along with the 2004 allocation. If you
choose not to make a withdrawal at this time, or miss the June 14 deadline, the next
payment date will be July 15, 2004. In order to be mailed a check on July 15, 2004,
you must have your application to us no later than July 3, 2004.
Withdrawal forms may be faxed or mailed into the offices.
Due to the overwhelming amount of calls regarding verification of receiving
the forms in the offices, you will need to mail it "return receipt requested" if
you want verification of arrival by mail. Ask your local post office for assistance
with this service. Faxing the form will be at your own risk.
The total Member Benefit Timber Distribution allocation for 2004 is $500 for
each member.
Please call the 1-800-759-4814 to receive information regarding the distribu
tion or if you did not receive a withdrawal form, you may request one. This will
be checked daily. If you have any questions regarding the application process
or your beneficiary, you can contact Liz Leno at 1-800-422-0232, extension 2082,
