Ways you can avoid Denials
from Contract Health Services
Always call in appointments at least 3 to 7 days before your appointment.
Use the after hours clinic on weekends and after regular business hours,
instead of the hospital emergency room.
Call in emergency services within 72 hours after being seen.
Use Grand Ronde Health & Wellness Center or Chemawa Clinic for routine
physical examinations, PAPs, well child exam and immunizations.
If you live in the Grand Ronde, Willamina or Sheridan areas, call the Grand
Ronde Health & Wellness Center answering service after hours to receive infor
mation on whether or not you should be seen in the emergency room 503-879-2002
or 1-800-775-0095. If you do not live in this area, you may also use this
number to help you determine if you are having an actual emergency or a con
dition that you may be able to see your regular physician for.

GOALS:
Proposed Grand Ronde Cemetery Rules September 25, 2001
1. Any Tribal member, their parents, their children, adopted children and spouses
can be buried in the cemetery. Other requests, will be considered on a case by
case basis and must be approved by Tribal Council.
2. After contacting Tribal Enrollment to verify eligibility, a family representative
should contact the Cemetery Manager to discuss burial arrangements.
3. All cemetery activities shall be prearranged with the Cemetery Manager prior
to the event.
4. All cemetery activities are closed to the media.
5. Families should contact the Cemetery Manager to reserve the cemetery build
ing for services or other cemetery activities.
6. Opening and closing the grave will be arranged through the funeral home.
Any exception, should be arranged with the Cemetery Manager and supervised
by staff.
7. All graves will have concrete liners or concrete vaults.
8. No driving on the grass.
9. Grand Ronde Cemetery plots can be reserved for future burial of family mem
bers by contacting the Cemetery Manager. Any disputes over burial location will
be resolved by the Cemetery Manager.
10. All requests for restoration work on old stones should be given to the Cem
etery Manager. Stone restoration services will be to provided to the extent that
Tribal funds are available. Condition of the stone will determine priority.
11. While staff will regularly prune trees and shrubs, the general rare and main
tenance of each grave site is the responsibility of the individual family. Plants are
an essential part of every cemetery. Many plants should be considered as historic
as the markers they shade. However, trees and large shrubs can crack or cause
the toppling of stones. When choosing a plant, family members are encouraged
to consider the size at maturity and potential root growth. If a gravestone is
currently being endangered by the growth of a plant, family members are encour
aged to try moving the plant a few feet or to take cuttings to replant after
removing the original shrub.
12. The cemetery will be opened from 7 a.m. until dusk 7 days a week.
Cultural Resources would like Tribal member comments
on the cemetery rules listed above.
If you have any suggestions or comments, please send them to CTGR Cemetery
Project, co Cultural Resources, 9615 Grand Ronde Road, Grand Ronde, OR 97347;
or you may call them at 1800-422-0232, ext. 4639. Thank You.
