A Day 111 the Life - The Joey Holmes exhibit at the A.C. Gilbert House in
Salem occupies an entire wall. The display shows photographs of Joey (the son of
Nancy and Tim Holmes) dancing at a pow-wow in full regalia as well as with family and
friends. Holmes Is an Athabaskan Tribal member from Nondaltan, Alaska who lives in
Grand Ronde with his parents. The bilingual exhibit also features placards describing
Joey's travels, daily life and Native heritage.

Effective September 1, 2001, you will be enrolled in the ODS Health Plans
Preferred Provider Option (PPO) medical plan. In order to receive the high
est level of benefits under a PPO medical plan, members must seek services
from providers who participate on the PHN network. If you seek services
from participating providers, your current level of coverage will increase for
most services from 80 percent to 90 percent.
If there are no participating PIIN providers in your area or you
elect to receive services from non participating PHN providers, your
benefits after September 1, 2001 will not be lower than your current
coverage.
The most recent listing of PHN participating providers is available to you
online at www.odshealthpIans.com. For a hard copy listing of the participat
ing providers in your area, please contact Val Sheker-Robertson or an ODS
Customer Service Representative toll-free at 1-877-605-3229.
Your new group number will be M067. You will be receiving new medical
member handbook and medical identification cards in the mail soon. If you
have not received your new card, please call Val Sheker at 503-879-2221 or
Amy Justen at 503-879-2223.
Prescription Drug Enhancements
The below listed prescription drug changes apply only to
Tribal members not their non-member dependent spouses.
Effective September 1, 2001, Tribal members will have a combined medical
and prescription drug card that will provide point of sale benefits for pre
scriptions purchased at participating retail pharmacies.
Beginning September 1, 2001, Tribal members will need to present
their combined medicalprescription drug ID card to a participating
pharmacy when picking up your prescriptions.
Tribal members will pay 20 percent of the cost of the drug, and the participat
ing pharmacy will do the rest. If prescriptions are purchased at a non-participating
pharmacy, Tribal members will pay for the prescription in full and sub
mit a claim to ODS Health Plans for reimbursement (current procedure).
In addition to the newly added prescription drug card program, effective
September 1, 2001 Tribal members will be able to receive up to a 90 day
supply of maintenance drug medications through ODS Health Plans' mail
order. The plan will pay 80 percent of the cost of the 90-day supply. Tribal
members can enroll in the mail order benefit at any time by completing the
form and mailing it to the address indicated or by enrolling on-line at
www.odshealthplans.com.
