ORDER GRANTING NAME CHANGE
Based on the allegations set forth in the Peti
tion For Change of Name and New Birth Certifi
cate Juvenile filed by the petitioner in the above
entitled case, and based on the Court's examina
tion of the Applicant, the Petitioner and other in
terested parties at the Show Cause Hearing held
on July 15, 1999.
THE COURT FINDS THAT:
1. Petitioner is over 18 years of age and is the
parent or legal guardian of Applicant, a minor
child;
2. Petitioner is an enrolled member of the Con
federated Tribes of the Grand Ronde Community
of Oregon;
3. Petitioner has not been granted a change of
name by the Tribal Court within the past twelve
(12) months;
4. All persons entitled to notice have been prop
erly served with a copy of the Notice To Parents
and Legal Guardians;
5. No objection to the petition has been filed by
any parent or legal guardian of the minor child
and no objection was raised at the Show Cause
Hearing; and- - - -
6. The proposed name change is in the best in
terests of the minor child.
THEREFORE, BASED ON THE FOREGO
ING FINDINGS, THE COURT ORDERS:
1. That the name of the Applicant be changed
from Caleb Alexander Tomasello to Caleb
Alexander Freauff;
2. That the Clerk of the Tribal Court shall cause
a copy of this Order to be published in the Tribal
Newsletter;
3. That the Clerk of the Tribal Court shall sub
mit a certified copy of this Order to the Tribal
Enrollment office;
4. That the Clerk of the Court shall provide peti
tioner with a certified copy of this Order to be sub
mitted by Petitioner to the Registrar of Vital Sta
tistics in the state in which the applicant
was born;
5. That upon receipt of the certified copy of the
Order, the Registrar shall issue a new birth cer
tificate of the Applicant to reflect the new name
of the Applicant as stated in this Order and shall
change any designation of race or national origin
on the birth certificate to reflect that the Petitioner
is an American Indian;
6. That the Petitioner, on behalf of the minor
child, shall surrender all old Tribal Identification
Cards bearing Applicant's former name to the
Tribal Enrollment Office prior to the Enrollment
Office issuing Applicant new Tribal Identification
Cards bearing the Applicant's new name; and
7. That within sixty (60) days from the date of
this Order the Tribal Enrollment staff, and, if
necessary, the Tribal Enrollment Committee, shall
cause new Tribal Identification Cards to be issued
to Applicant showing the new name of the Tribal
Member and shall change the official member
ship roll and all official enrollment records of the
Tribe to reflect the Applicant's new name as stated
in this Order.

Arthelia Mae Clark has battled health
issues but loves her life and family.
Arthelia's parents were David Holmes and
Vida Mitilda (Smith) Holmes. She came
. from a family of five brothers and 3 sis
ters her living sisters are Vivian Martin and
Priscilla Hofenbredl; and two brothers are Jessy
"Benny" Bahr and Loren F. Holmes.
Arthelia was married 23 years to Elton Clark
before losing him in 1978. She has two sons,
David Clark and John Clark; one daughter,
Arlettia (Clark) Krehbiel; eleven grandchildren
and one great-grandchild.
Arthelia was born a half mile from where she
currently lives today. She lives in the house her
dad and husband built in 1955. Thanks to the
Bureau of Indian Affairs and the Tribe's Hous
ing Program, Arthelia has been able to remain in
her house. Without their help, the house would
have fallen long ago.
Arthelia's hobbies include listening and sign
ing to country western music, going on picnics,
cooking and visiting relatives. Other interests
include making crafts at the monthly "Blind and
Handicapped Workshop. ' Earlier in life, Arthelia
was a wealth of knowledge about her family
she could tell you all her aunts, uncles, cousins
and their birthdays.
She liked to watch her dad shoe horses, build
houses, and listen to him play the fiddle. She
loved to watch her mom sew quilts and clothes,
crochet, grow flowers and a vegetable garden. She
walked wherever she went as a young girl, and

also liked to go swimming in the creek just be
hind her house.
With her failing eyesight, she struggled through
school. After graduating from 8th grade, she
stayed home and helped out with the chores.
After she married, raising her family was her
priority. She kept a clean house and raised her
three children.
Because her eyesight continued to fail, her dad
and aunts Kate and Hattie tried to do a "Indian
healing dance and chant" to stop her' increasing
blindness. Several cataract surgeries never helped
either. After severe chronic migraine headaches,
local doctors sent her to O.H.S.U. in Portland. She
was found to have "sinus cancer." Doctors tried
to stop the cancer with kemotherapy for a total of
78 treatments. The kemo helped but wasn't
enough, and surgery was needed. The cancer
was stopped, but changed her life immensely.
"Mom is such a sweet, honest, gentle loving,
caring, giving, kind and God-respecting woman;
after so much suffering on earth, I know there's
a red carpet rolled out for her when she gets into
heaven," said her son, John Clark.
A piece of advice Arthelia would give youth: "Do
the best you can and be good to one another. "
REQUEST FOR PROPOSALS
A statement of services requested by the Confederated Tribes of the Grand
Ronde Community of Oregon in this Request for Proposals
The Confederated Tribes of the Grand Ronde
Community of Oregon (Tribe) seek proposals from
any parties interested in providing building depart
ment services for the Tribe under the auspices of
a public agency. The majority of the services in
volve administration of local building codes identi
fied by the Tribe and making sure that tribal devel
opments comply with the identified codes.
The identified codes include:
The Uniform Building Code (U.B.C.)
The Uniform Plumbing Code (U.P.C.)
The Uniform Mechanical Code (U.M.C.)
The National Electrical Code (N.E.C.)
The Life Safety Code (N.F.P.A. 101)
Oregon Manufactured Dwelling Standards
(O.M.D.S)
Other codes may apply. Any additional codes
would be alerted to the Tribe and recommended
by the selected building department service pro
vider A full RFP will be sent to interested parties. Pro
posals must be received no later than 2:00 P.M.
(local time), on Tuesday, August 31; 1999. Re
spondents who wish to submit proposals by mail
can do so at their own risk. The Tribe assumes
no responsibility for delayed or undelivered mail
or express packages. Proposals may be hand
delivered to the Confederated Tribes of Grand
Ronde Governance Center at the Governance
Center Reception Counter. It is the responsibility
of the proposal delivery service provider to lo
cate the correct building and reception counter
before the submittal deadline. Proposals which
are not delivered by the above specified time,
date, and location will not be considered.
