JUNE 15, 1999 11 Changes made to home repairs; down payment policies Recent changes have been made to the polices for home repairs and down payment grants for the 1999 program year. The programs now available are: Minor Medical Adaptation Grant: This is a program that provides grants to Grand Ronde Tribal mem bers for medically required access ramps, grab bars and other minor accommodations for physical limita tions. There is no income restriction to qualify. The property can be lo cated anywhere in the continental United States. The maximum grant amount is $3,000. Home Repair Grant: This program provides grants to low income tribal homeowners for basic repairs to roofs, heating, plumbing, electrical, foundations, n Grand Ronde Tribal Housing Authority and also includes repairs to wood damage and installation of weath erization items such as insulation. Income limits are based on family size. The maximum annual income for a family of four is $38,250. The property must be located in the Housing Authority's Ten County Service Area. The maximum grant amount is $10,000 ($3,000 for mo bile homes built prior to 1977). Travel trailers and RVs are not eli gible. Recipients must maintain the home as their primary residence for a five-year period from the date of the grant, or pay back the grant funds in full. Down Payment Grant: The Down Payment Grant pro vides a grant of up to $10,000 to "first time" homebuyers for assis tance with the down payment on the purchase of a single family home. First time homebuyers are those who have not owned a home in the past three years prior to application. Gross annual income must be be tween $15,000 - $65,000 to qualify, and be adequate to qualify for fi nancing from a lending institution. Applicants must also complete a homebuyer education class. The property to be purchased may be lo cated anywhere in the continental United States. The recipient is re quired to contribute $1,000 toward the closing in one of several forms. As with the Home Repair Grant, re cipients must maintain the home as their primary residence for a five year period from the date of the grant, or pay back the grant funds in full. For all programs, priority will be given based on date of application, with preference given to Elders and people with disablities. Diabetes: two special projects available The Tribal Wellness Program cur rently has two special projects avail able for members with diabetes. Both projects are very limited in funding and will be honored on a first come, first serve basis. We have Advantage blood glucose monitors available. To be eligible to receive one you will need: 1) to be a tribal member, 2) be diagnosed with diabetes by your doctor and have a doctor's prescription, 3) have an old monitor that does not work and, 4) be willing to submit a month of glu cose reading to the Tribal Wellness Program. Several staff and a few tribal mem bers have completed the Diabetes Education class that is provided through Salem Hospital. This is a four-day, comprehensive class that enables patients with diabetes and their spouses to "Live Well with Dia betes." The daytime hours for this class are 8:30 - 2:30 and the evening classes are 5:30 - 9:30. A person can attend either day or evening class. It is important for anyone interested in taking this class to be committed to the whole class schedule because every part of the class is important to learning a complete picture about mm CASE NO. The Petitioner, Ryan Mathew Freauff, on behalf of the Applicant, Caleb Alexander Tomasello, a minor child, has petitioned the Court for a Change of Name for the minor child. Applicant allegedly desires to change his name to Caleb Alexander Freauff, for the reason stated that: Applicant's last name differs from the last name of the parent and he wishes to assume the last name of the parent. Based on the foregoing petition filed with the Court on April 19 , 1999, and good cause appearing therefor, IT IS ORDERED THAT the petitioner and any person objecting to the grant ing of the petition shall appear in the Tribal Court of the Confederated Tribes of the Grand Ronde Community of Oregon, located in the Governance Center, Room 106 at 9615 Grand Ronde Road, Grand Ronde, Oregon, on July 15, 1999 at 10:00 a.m. to show cause why the Court should not enter judgement granting the proposed change of name. IT IS FURTHER ORDERED that any written response to this Order by any party to this case shall be filed on or before July 9, 1999. The Clerk of the Tribal Court shall cause a copy of this Order to be served upon all parties in accordance with Rule 14 of the Rules of Pleading, Practice and Procedure of the Tribal Court of the Confederated Tribes of the Grand Ronde Community of Oregon and file proof of service with the Court on or before the date set for hearing in this matter. There is a Diabetic Support group meeting at the Grand Ronde Health & Wellness Center every third Tues day of the month at 11:00 am. The group has been very busy with vari ous topics which are selected by the members of the group. A snack and recipe are shared so that we gener ate new ideas for the family menu.
