Logo Contest
If you are a tribal member with a special
artistic ability, please consider submitting
an original logo to represent the 1 997 Grand
Ronde Pow-wow. Logos must be original
and representative of what the Grand Ronde
Pow-wow means to our tribe. All submis

Benefit Fund notice
If you plan to make a withdrawal from
your Member Benefit Fund in the near fu
ture, please note that all applications must
be received by the last day (the 30th) of each
month in order to be processed by the 10th
day of the following month. All checks will
be mailed on the 10th. This is regarding
the Member Benefit allocated in Septem
ber of 1996 only.

The last day of the extended period to
enroll in the Tribal Member Health Plan
was January 15, 1997. We received many
more applications than we initially ex
pected, a total of 1766! Those tribal mem
bers who submitted their applications to
us by December 1 3 were enrolled into the
plan by January 1 . Applications received
in our office by January 15 will be en
rolled in the plan effective February 1.
D ABOUT ODS HEALTH PLAN B
Our next open enrollment period for
the ODS plan will be in May and June
for an eligibility date of July 1, 1997.
B If you are (or will be) enrolled in the
ODS plan and are expecting a baby in
1 997, please note that your newborn will
be covered for a period of thirty (30) days
under your plan. Following the 30 day
period, coverage for the newborn will be
dropped until he or she receives an en
rollment number through the Enrollment
Office of the Tribe. Your new baby will
be eligible for coverage under the ODS
plan as soon as an enrollment number is
assigned. If you have questions about the
enrollment process, contact Margo Mer

B If you have enrolled in the plan and
have a January 1, 1997 effective date, but
have not received your ODS Identifica
tion Card in the mail, please contact ODS
at (503) 228-6554 or toll-free at 1-800-852-5195.
B If your enrollment in the plan becomes
effective on February 1, 1997, you can
expect to receive your ODS Identification
Card in the first two weeks of February.
B If you have coverage questions about
the plan or need any other information

tive from the Tribe's Risk Management
Department would be happy to assist you.
B The following people signed up for the
