Health plan update
Thanks to all tribal members who re
sponded to the initial application process
for the Tribal Member Health Plan. We re
ceived over 1300 applications! Tribal mem
bers who turned in completed applications
by December 13 will be enrolled effective
January 1, 1997. ODS plan summaries and
benefit cards will be mailed to these plan
enrollees sometime during the first week in
January. If you haven't received your plan
summary or medical card by January 15,
please call Val or Tina in the Risk Manage
ment Department at 879-2221 or 1-800-422-0346.
For those tribal members who are still
interested in signing up for the ODS health
plan but missed the December 13 deadline,
we will be offering a one time extended
enrollment date. If we receive your ap
plication on or before January IS, you
will be enrolled into the health plan ef
fective February 1, 1997. Please return
your completed application to the Risk
Management Department, 9615 Grand
RondeRoad, Grand Ronde, OR 97347. Re
member these applications must be received
in our office no later than 5 p.m. on Janu
ary 15. Applications received after Janu
ary 15 will be held until the next enrollment
period for a July 1, 1997 start date.

Elder housing survey coming soon
The Tribal Housing Authority is considering the construction of a 30 unit Assisted Liv
ing Facility (ALF) for Elders. This would be included as part of a larger Elder develop
ment scheduled to begin construction in 1 997 on Tribal land in Grand Ronde. This facility
would serve Elders who may need regular or continuous support and assistance with daily
routine living tasks such as cooking, dressing, bathing, taking medication, mobility, etc.
The Housing Authority needs to be able to show that there is a need and desire on the part
of tribal Elders for such a facility. If this is proven, they will determine if such a project is
financially feasible and begin the design process and search for financing.
In January, a survey will be sent to all Elders and will focus on questions regarding this
type of housing. Therefore, it will be v ery important that all Elders who arc interested
in such housing for themselves now or at some time in the future respond to this
survey. We do not know how great the need is, and so in order to proceed with such a
project we need your input as the first step!
Elk leg basket raffle
The Culture and Heritage Board will be
selling raffle tickets to help with the costs
of the Elders' Social planned for January, as
well as the Tribal Quilt Project. The draw
ing for the winner will be at the February
General Council meeting, and the prize is
elk leg basket made by tribal employee
Vivian DcMary. You need not be present
at the meeting to win. To purchase tick
