Please call the Health Clinic for an appointment. If you bring in a child between the ages of 6 months to three years, we will need their immunization cards to complete the visit. The tuberculosis skin test must be read by a health professional 48-72 hours after application for it to be valid. Let's do our part to keep tuberculosis from spreading! The Honor Lights serve as a visual reminder of those you wish to honor during the coming holiday season. We invite you to join symbolically in this hon oring event. The cost of an Honor Light is $1. Donations will also be accepted. The proceeds of this fund-raising event will go towards pur chasing the Art Work that will adorn the Health & Wellness facility which will be opening dur ing the summer of 1 997. We have received over 1,000 applications in just three weeks! For many reasons, we have been asked to provide an extension for this initial sign up period. This has been arranged through ODS. For those applications postmarked after December 13, and for those people who have not yet turned in an application, an extension has been granted until January 15, 1997 for a revised start date of February 1, 1997. For those individuals who have submitted their applications by the December 1 3 dead line, your plan will commence January 1 , 1 997. You should be receiving your coverage packet and health insurance card in the mail by January 5, 1997.
