She will receive the Allotee Scholarship award each year until she earns her degree. Angie is also the recipient of the Grand Ronde Tribal Edu cation Trust Fund Scholarship in the amount of $5,000. Angie will be entering her second year as a graduate stu dent at Lewis & Clark School of Law in Portland, Oregon. Elaine LaBonte-Moon has been selected as the recipient of a National Aeronautics Scholarship Association Deadline to apply to Education Programs 199596 Academic Year Fall TermSemester 1 995 is as follows: Higher Education Pro gram - 24 students; Adult Vocational Training Program - 8 stu dents; Education Trust Fund Scholarship - 23 students. Higher Education Program: Fall TermSemester - July 1, 1995 Winter TermSpring Semester - November 1, 1995 Spring Term - February 1, 1996 Adult Vocational Training Program: Open enrollment, dependent upon determination of eligibility if enrolled in a propriety school (usually takes 30 to 45 days). Deadline dates above apply to those who plan to attend a Com munity College. Education Trust Fund Scholarship Program: April 30, 1996 for the 199697 academic year. Adult Vocational Training Program The Adult Vocational Training Program will now accept applica tions for enrollment from Tribal members who reside out of the six county service area. Due to limited funding, only three eligible ap plicants who live out of the service area, will be awarded funding during the 199596 academic year. Shelley is also the recipient of the Grand Ronde Tribal Edu cation Trust Fund Scholarship in the amount of $5,000. Shelley is entering her second year of graduate studies at Willamette University, Atkinson School of Business, in Salem, Oregon. Shelley will graduate in June of 1996 with a Master's Degree in Business. All students are responsible for having their grade reports sent to the Tribal Education Program upon completion of each term or semester. Either make arrange ments with the Registrar's Office to have the reports sent directly to the Tribal Office or mail a copy of your grade report to the Tribal Office yourself. 2. Students, please notify the Education Program of address and telephone number changes. 3. If emergencies occur that effect your attendance, notify the . You must maintain a 2.00 GPA and complete a minimum of 1 2 credit hours per termsemester to be eligible for assistance. 5. Students who are placed on PROBATIONARY STATUS will not be awarded the following term until they have provided the Edu cation Program an OFFICIAL COPY of their transcripts. Tran scripts must include the term of probation and must show success ful completion of 12 credit hours earned with at least a 2.00 GPA. No award will be made until tran scripts have been submitted. 6. Continuing students must file their Federal Financial Aid Forms by February 15, 1996. You must provide a copy of your application to the Education Attention Students! Reminder: All Tribal Education Program students need to submit a copy of class registration to the Tribal Education Program at the begin ning of each termsemester. Program for filing verification. 7. All students must sign a "Re lease of Information" form regard ing attendance, financial aid tran scripts, education budgets, progress, and grade reports. 8. It is your responsibility to remain in "good academic stand ing." If a student demonstrates an "on againoff again" pattern of being placed on PROBATION ARY STATUS, they will be placed on Probationary Status for one complete academic year. Failure to meet minimum aca demic requirements during this period will result in SUSPEN SION. 9. Students who withdraw from school due to medical or health related reasons must obtain a Statement of Verification from their physician or health care pro-, vider identifying need to withdraw from school. Students must also obtain a Statement of Release from their physician that identifies ability to return to school. Copies of phy sicians statements must be pro vided to the Education Program. 10. Students entering their SE NIOR year are required to have a GRADUATION AUDIT EVALUATION completed fall term and provide a copy of the AuditEvaluation to the Tribal Education Program.
