Gerritz, M.D. Tuberculosis is on the rise All new employees need a tuberculosis skin test prior to hire and yearly skin tests thereafter. However, if you work in a high risk area such as Health, Dental, Wellness, Chemical Dependency, Tribal Family Action, and Senior Meal Site they will need a tuberculosis skin test every six months. Please call the Grand Ronde Medical Clinic now at 1-800-775-0095 or 879-2002 and make an appointment for a tuberculosis skin test. Medical history is important to Clinic In order to get a clearer and more comprehensive medical history of you, we are asking you to spend a few minutes in the health clinic giv ing us information concerning your medical history, such as medication (please bring all your medicine bottles into clinic when coming for an appointment) allergies, previous hospitalizations, operations and seri ous illnesses. With this information we will be able to provide better quality medi cal care to you. Any provider (nurse or doctor) could pick up your chart and know something about your history prior to seeing you in the health clinic. This allows us to treat you better; also it will allow us to obtain a top-notch seal of approval in being an outstanding Tribal Clinic. USD needs volunteers The Grand Ronde Human Ser vices Division is offering a $50 prize for one man and one woman to participate in a health assess ment survey in the community from April through June. Participants will be randomly selected from the tribal member ship by enrollment number. Those individuals who are cho sen will be eligible to enter in the drawing.
