Health survey arriving soon
Dear Tribal Members,
During the month of May, each tribal
member will be receiving a I lealth Insur
ance Questionnaire from the Tribe. The
purpose of the questionnaire is to gather
information on what current health care
coverage and benefits members and their
households have. The information is im
portant in order to identify unmet health
care needs and to develop programs and
benefits to meet these needs.
The health planning we are conduct
ing is part of the strategic planning pro- I
cess adopted and begun in 1994. In re
gards to health care, members expressed
support for having at its center, "healthy
families". In addition, members wanted
to ensure that all members equally re
ceived benefits, regardless of residence.
With these goals in mind, tribal staff and
the Tribal Council are working to iden
tify the most cost effective and efficient
manner for members to access health care.
The information we collect from the ques
tionnaires will help us greatly with plan
ning. Only one questionnaire needs to be
completed per household, but copies
were sent to each member to insure that
no one is left out of the survey process. It
will include a postage paid envelope to
return it in. Please return by May 1 5,
1995.
