DUTIES: Direct activities toward specialized treatment and rehabilitation as follows: -Provide client assessments that shall include personal history, alcohol abuse history, physical, social psycho logical, and spiritual evaluations; -Develop and implement individualized comprehensive alcoholism treatment plans based on the assessment; -Implement specific individual and group counseling sessions for the client and hisher family; -Utilize the various community resources as may be needed to assist the client in plans for education, vocational training, employment or housing; -Help client schedule activities for personal, social, and emotional rehabilitation; -Coordinate and serve as liaison with courts, probation officers, andor social services agencies as needed; -Maintain all patient records in accordance with the policies of the Tribe and Indian Health Services; -Perform counseling duties in all phases of out-patient counseling; -Maintain and submit all required reports and forms; -Other duties as assigned.
